पिें नभोगी कल्याण जवभाग) अजधसचू ना नई दिल्ली, 16 िुलाई, 2024 सा.का.जन. 410(अ).— राष्ट्रपजत, संजवधान के अनुच्छेि 309 के परंतुक और अनुच्छेि 148 के खंड (5) द्वारा प्रित्त िजियों का प्रयोग करत े हुए और भारतीय लेखापरीक्षा एव ं लखे ा जवभाग म ें कायिरत कार्मिकों के संबंध म ें भारत के जनयंत्रक और महालेखापरीक्षक स े परामिि करने के पश्चात,् केंद्रीय जसजवल सेवा (पेंिन) जनयमावली, 2021 म ें और संिोधन करने के जलए एतिद्वारा जनम्नजलजखत जनयम बनाते ह,ैं अथाित ् :- 1. सजं क्षप्त नाम और प्रारंभ.- (1) इन जनयमों का संजक्षप्त नाम केंद्रीय जसजवल सेवा(पेंिन) संिोधन जनयम, 2024 ह।ै (2) य ेरािपत्र म ें इनके प्रकािन की तारीख स ेएक सौ पच् चीस दिनों के पश्चात् प्रवृत्त होंगे। 2. केंद्रीय जसजवल सेवा (पेंिन) जनयमावली, 2021 (जिसे इसम ें इसके पश्चात ् उि जनयम कहा गया ह)ै के जनयम 50 के उपजनयम (15) के खंड (ङ) के स्ट् थान पर जनम्न जलजखत खंड रखा िाएगा, अथाित् :- ‘‘(ङ) सरकारी कमिचारी सरकारी सेवा स े सेवाजनवृत् त होने के समय, प्ररूप 6-क म ें कुटुंब के अद्यजतत ब् यौरे पुन: प्रस्ट् तुत करेगा।’’ 3. उि जनयमों के, जनयम 53 म,ें - 4294 GI/2024 (1)2 THE GAZETTE OF INDIA : EXTRAORDINARY [PART II—SEC. 3(i)] (क) उपजनयम (1) के स्ट्थान पर जनम्नजलजखत उपजनयम रखा िाएगा, अथाित ् :- “(1) िब तक दक सरकार के साधारण या जविेष आििे द्वारा अन्यथा छूट न िी गई हो सरकारी कमिचारी के पेंिन मामल े पर ऑनलाइन मोड (भजवष्य/ई-एचआरएमएस) के माध्यम स े कारिवाई की िाएगी और सेवाजनवृत्त होने वाले पिाजधकारी िो ई-एचआरएमएस पर ह,ैं केवल ई-एचआरएमएस (अजधवर्षिता मामलों म)ें के माध्यम स े प्ररूप 6-क प्रस्ट् तुत करेंगे। सेवाजनवृत्त होन े वाले कमिचारी िो ई-एचआरएमएस पर नहीं ह ैं तथा अजधवर्षिता स े जभन् न सभी मामलों म ें (अथाति ् स्ट् वजै च्छक सेवाजनवृजत्त, समयपूव ि सेवाजनवृजत्त, राज्य सरकार/सावििजनक क्षत्रे के उपक्रम/स्ट् वायत् त जनकाय म ें स्ट्थ ायी आमेलन, अिक् तता, अजनवाय ि सेवाजनवृजत्त तथा सेवा स े पिच् युजत या हटाया िाना), प्ररूप 6-क भजवष्य के माध्य म स े प्रस्ट्त ुत करेंगे।”; (ख) उपजनयम (2) के खडं (क) और खडं (ख) म ें “भजवष्य” के स्ट्थान पर जनम्नजलजखत रखा िाएगा, अथाति ् :- “ऑनलाइन मोड (भजवष्य/ई-एचआरएमएस)”। 4. उि जनयमों के, जनयम 57 म,ें - (क) उपजनयम (1) के खडं (ग) म ें उपखंड (iii) के स्ट्थान पर जनम्नजलजखत उपखंड रखा िाएगा, अथाित ् :- “(iii) प्ररूप 6-क प्रस्ट्तुत करने के जलए सेवाजनवृत्त होने वाल े सरकारी कमिचारी को सलाह”; (ख) उपजनयम (2) म ेंखंड (क) के स्ट्थान पर जनम्नजलजखत खंड रखा िाएगा, अथाित ् :- “(क) सरकारी कमिचारी अपनी सेवाजनवृजत्त की तारीख स े छह मास पूवि कायािलयाध्यक्ष को सम्यक रूप से भरा हुआ प्ररूप 6-क प्रस्ट्ततु करेगा।”; (ग) उपजनयम (3) के खंड (क) और खंड (ख) म ें क्रमि: “प्ररूप 4 और प्ररूप 6” के स्ट्थान पर “प्ररूप 6-क” रखा िाएगा; (घ) परंतुक के स्ट्थान पर, जनम्नजलजखत परंतुक रखा िाएगा, अथाित् :- “परंत ु िहां उि प्ररूप पजत-पत्नी या कुटुंब के दकसी अन्य सिस्ट्य द्वारा प्रस्ट्तुत दकया िाता ह,ै सरकारी सेवक तब तक पेंिन के प्रजतित को सरांिीकृत कराने के लाभ का हकिार नहीं होगा िब तक दक वह केंद्रीय जसजवल सेवा (पेंिन का सरांिीकरण) जनयमावली, 1981 के अनुसार ऐसे सरांिीकरण के जलए बाि में स्ट् वय ं आवेिन नहीं करता।” 5. उि जनयमों के, जनयम 58 म,ें - (क) उपजनयम (1) के स्ट्थान पर जनम्नजलजखत उपजनयम रखा िाएगा, अथाित ् :- “(1) ऐसा सरकारी सेवक िो, अजधवर्षिता स े जभन्न कारणों स े सेवाजनवृत्त हो रहा हो या हुआ हो, यथाजस्ट्थजत ऐसी सेवाजनवृजत्त के सक्षम प्राजधकारी द्वारा अनुमोिन के या ऐसी सेवाजनवृजत्त के प्रभावी होन े अव्यवजहत पश्चात् कायािलयाध्यक्ष को प्ररूप 6-क प्रस्ट्तुत करेगा।”; (ख) उपजनयम (2) के खंड (क) और खंड (ख) म ें“प्ररूप 4 और प्ररूप 6” के स्ट्थान पर क्रमि: “प्ररूप 6-क” रखा िाएगा। (ग) परंतुक के स्ट्थान पर जनम्नजलजखत परंतुक रखा िाएगा, अथाित् :- “परंत ु िहा ं उि प्ररूप पजत-पत्नी या कुटुंब के दकसी अन्य सिस्ट्य द्वारा प्रस्ट्तुत दकया िाता ह,ै सरकारी सेवक तब तक पेंिन के प्रजतित को सरांिीकृत करान े के लाभ का हकिार नहीं होगा िब तक दक वह केंद्रीय जसजवल सेवा (पेंिन का सरांिीकरण) जनयमावली, 1981 के अनुसार ऐस े सरांिीकरण के जलए बाि म ें स्ट् वय ंआवेिन नहीं करता।”[भाग II—खण् ड 3(i)] भारत का रािपत्र : असाधारण 3 6. उि जनयमों के, जनयम 59 म,ें - (क) उपजनयम (1) म ें“प्ररूप 4 और प्ररूप 6” के स्ट्थान पर क्रमि: “प्ररूप 6-क” रखा िाएगा; (ख) उपजनयम (2) म ें“जनयम 58 म ें जनर्िष्टि प्ररूपों” के स्ट्थान पर “जनयम 58 में जनर्िष्टि प्ररूप” रखा िाएगा। 7. उि जनयमों के, जनयम 60 म,ें - (क) उपजनयम (1) के खडं (i) म ें “प्ररूप 4, प्ररूप 6 और फॉमेट 9 म ें बैंक को वचनबंध की प्रजतयां” के स्ट्थान पर “प्ररूप 6-क” रखा िाएगा; (ख) उपजनयम (3) म,ें “प्ररूपों म ें स े प्रत्येक की एक प्रजत” के स्ट्थान पर “प्ररूप और फॉमेट की प्रजतयां” रखा िाएगा; (ग) उपजनयम (4) म,ें “प्ररूप 4 और प्ररूप 6” के स्ट्थान पर “प्ररूप 6-क” रखा िाएगा। 8. उि जनयमों के, जनयम 62 के उपजनयम (6) के खडं (क) म ें “प्ररूप 4 और प्ररूप 6” के स्ट्थान पर “प्ररूप 6-क” रखा िाएगा। 9. उि जनयमों के, जनयम 63 म,ें उपजनयम (4) के खडं (क) और खंड (ख) म ें “फॉमेट 9” के स्ट्थान पर “प्ररूप 6-क” रखा िाएगा। 10. उि जनयमों के, जनयम 80 के उपजनयम (5) म,ें (i) परंतुक के स्ट्थान पर जनम्न जलजखत परंतुक रखा िाएगा, अथाित् :- ‘‘परंतु यदि सरकारी कमिचारी की मृत् य ु होन े पर कुटुंब का कोई सिस्ट् य कुटुंब पेंिन प्राप् त करन े के जलए पात्र नहीं ह,ै तो कुटुंब का ऐसा सिस्ट् य जिसके पक्ष म ें सरकारी कमिचारी द्वारा उपिान के संिाय के जलए नामजनिेिन दकया गया था, को प्ररूप 10 के स्ट्थ ान पर प्ररूप 6-क िमा करन े की अनुज्ञा िी िाएगी और कुटुंब के उक्त सिस्ट् य को प्ररूप 6-क म ेंअपने बैंक खाते का जववरण िेना होगा।’’ (ii) खंड (ग) के स्ट्थान पर जनम्नजलजखत खडं रखा िाएगा, अथाति ् :- “(ग) कायािलयाध्यक्ष यथाजस्ट्थजत प्ररूप 4, प्ररूप 10, फामेट 9 और फामेट 13 (यदि लाग ू हो) या प्ररूप 6-क को प्ररूप 7 और फॉमेट 10 म ें एक अग्रेषण पत्र के साथ लेखाजधकारी को पेंिन, सेवाजनवृजत्त उपिान और कुटुंब पेंिन यदि लागू हो, के प्राजधकरण के जलए भेिगे ा।”; 11. उि जनयमों के प्ररूप 4 के पैरा 1 म ें“प्ररूप 6 के साथ” के स्ट्थान पर “प्ररूप 6-क म”ें म ेंरखा िाएगा। 12. उि जनयमों म,ें “प्ररूप 6” के स्ट्थान पर “प्ररूप 6-क” रखा िाएगा। 13. उि जनयमों के, प्ररूप 7 म ें “प्ररूप-7 सेवाजनवृजत्त िेयताओं पर समयोजचत कारिवाई हते ु कायािलयाध्यक्ष के जलए िांच सूची” उपिीष ि के अधीन, मि 9 और मि 10 का लोप दकया िाएगा। 14. उि जनयमों के, फॉमेट 10 म ें “संलग्नक” उपिीष िके अधीन,- (क) मि 2 के स्ट्थ ान पर जनम्न जलजखत मि खंड रखा िाएगा, अथाित ् :- ‘‘2. प्ररूप 6-क या प्ररूप 10’’ (ख) मि 3 के स्ट्थ ान पर जनम्न जलजखत मि खंड रखा जलजखत मि खंड रखा िाएगा, अथाित ् :- ‘‘4. सलं ग् नकों और िांचसूची सजहत जवजधवत रूप स े भरा हुआ प्ररूप 7 या प्ररूप 11’’ [फा. सं. 55/13/2023-पी&पीडब्ल्य(ू सी)] ध्रुवज्योजत सेनगुप्ता, संयुि सजचव प्ररूप 6-क प्रकार एकल/ पजत या पत्नी के साथ संयुि खाता संख्या बैंक का नाम िाखा का पता आईएफएससी रटप्पण 1: कृपया खाता धारक का नाम ििािने वाली पासबुक के पृथम पृष् ठ/रद्द दकए गए चेक/िस्ट्तावेि की एक प्रजत संलग्न करें। (बैंक खाते, इस फॉमि और कायािलय के ररकॉडि में नाम एक िैसा होना चाजहए।) रटप्पण 2: कृपया सुजनजश्चत करें दक सरकारी कमिचारी संयुि खाते में प्राथजमक खाताधारक है। रटप्पण 3: यदि कायािलय अध्यक्ष का यह समाधान हो िाता है दक सेवाजनवृत्त होने वाले सरकारी कमिचारी के जलए उसके जनयंत्रण से बाहर के कारणों से संयुि खाता खोलना संभव नहीं ह,ै तो इस आवश्यकता में छूट िी िा सकती है।[भाग II—खण् ड 3(i)] भारत का रािपत्र : असाधारण 5 4. सवे ाजनवत्तृ होन े वाल/ेसेवाजनवत्तृ सरकारी कमचि ारी की ओर स े इस प्ररूप को िमा करन े के जलए जनयम 57(3) के अधीन प्राजधकृत सरकारी कमचि ारी के कुटुंब के सिस्ट्य का ब्यौरा : नाम सरकारी कमिचारी के साथ नातेिारी आधार सं. राष् रीयता फ्लैट/मकान स.ं/जबलल्डंग का नाम गली/मोहल्ला ग्राम एवं डाकघर/ब् लॉक िहर एवं जिला राज् य जपन कोड मोबाइल नंबर िरू भाष सं.(यदि कोई हो) ई-मेल आईडी सरकारी कमिचारी द्वारा प्ररूप प्रस्ट् तुत नहीं करने का कारण 5. मैं अपनी पेंिन का % (िब्िों में) केंद्रीय जसजवल सेवा (पेंिन) जनयमावली, 2021 के अधीन केंद्रीय जसजवल सेवा (पेंिन संरािीकरण) जनयम, 1981 के उपबंधों के अनुसार संरािीकृत करने की वांछा करता ह।ं रटप्पण: कुटुंब का कोई सिस्ट्य जिस े सवे ाजनवत्तृ होन े वाले/सवे ाजनवत्तृ सरकारी कमचि ारी की ओर स े जनयम 57(3) के अधीन इस प्ररूप को िमा करन ेके जलए प्राजधकृत दकया गया है, पिें न के प्रजतित के संरािीकरण के जलए आवेिन करन ेके जलए पात्र नहीं होगा। 6. उपिर्िित करें दक क्या कुटुंब पेंिन दकसी अन्य स्रोत से भी अनुज्ञेय ह ै- सैन्य या राज्य सरकार या सावििजनक क्षेत्र के उपक्रम/स्ट्वायत्त जनकाय/केंद्रीय या राज्य सरकार के अधीन स्ट्थानीय जनजध : हां/नहीं 7. क्या सरकारी कमिचारी के जवरुद्ध कोई जवभागीय या न्याजयक कायिवाजहयां लंजबत है? यदि हां, तो तत्सबं ंधी ब्यौरा क्या है? 8. क्या सरकारी कमिचारी कायािलय अध्यक्ष के माध्यम से कायािलय में पेंिन संिाय आिेि (पीपीओ) प्राप्त करना चाहता है? हां/नहीं घोषणाए ं (िो लाग ूहो, उस कथन पर सही का जनिान लगाएं) (1) मैं जनयम 57(1)(ग) के अधीन कायािलय अध्यक्ष द्वारा यथा प्रज्ञाजपत पेंिन और उपिान के जलए गण्य अहिक सेवा की अवजध से संतुष्ट हं। या मैं जनयम 57(1)(ग) के अधीन कायािलय अध्यक्ष द्वारा यथा प्रज्ञाजपत पेंिन और उपिान के जलए गण्य अहकि सेवा की अवजध से संतुष्ट नहीं ह ं और मैंने इस संबंध में पृथक रूप से एक अभ्यावेिन प्रस्ट्तुत दकया ह।ै या मुझे पेंिन और उपिान के जलए गण्य अहकि सेवा की अवजध के बारे में सूजचत नहीं दकया गया ह।ै (2) मैं जनयम 57(1)(ग) के अधीन कायािलय अध्यक्ष द्वारा यथा प्रज्ञाजपत पेंिन और उपिान के जलए गण्य पररलजब्धयों और औसत पररलजब्धयों से संतुष्ट हं। या मैं जनयम 57(1)(ग) के अधीन कायािलय अध्यक्ष द्वारा यथा प्रज्ञाजपत पेंिन और उपिान के जलए गण्य पररलजब्धयों और औसत पररलजब्धयों से संतुष्ट नहीं ह ंऔर मैंने इस संबंध में पृथक रूप से एक अभ्यावेिन प्रस्ट्तुत दकया ह।ै या मुझे पेंिन और उपिान के जलए गण्य पररलजब्धयों और औसत पररलजब्धयों के बारे में सूजचत नहीं दकया गया ह।ै (3) मुझे ज्ञात ह ैदक कुटुंब पेंिन की प्रत्येक मंिूरी और उसे िारी रखने की एक जववजक्षत िति यह होगी दक िावेिारकुटुंब पेंिनभोगी का / आचरण भजवष्य में अच्छा बना रहे।6 THE GAZETTE OF INDIA : EXTRAORDINARY [PART II—SEC. 3(i)] संलग्न क : संलग्न सूची के अनुसार रटप्पण 1: पेंिन का संरािीकरण स्ट्वैजच्छक है। यदि सेवाजनवृत्त होने वाला सरकारी कमिचारी पेंिन के प्रजतित का संरािीकरण कराने का इच्छुक नहीं ह ैतो मि 5 को काट दिया िाए। रटप्पण 2: यदि सेवाजनवृत्त होने वाला/सेवाजनवृत्त सरकारी कमिचारी इस प्ररूप को िमा करने के बाि पेंिन के संरािीकरण के जलए आवेिन करने का इच्छुक हो, तो केंद्रीय जसजवल सेवा (पेंिन संरािीकरण) जनयमावली, 1981 के प्ररूप 1-क में सेवाजनवृजत्त पेंिन के संरािीकरण के जलए पृथक आवेिन करना अपेजक्षत ह।ै रटप्पण 3: अजनवायि सेवाजनवृजत्त पेंिन/अिि पेंिन/अनुकंपा भत्ता की ििा में एक वषि के पश्चात् पेंिन के संरािीकरण के जलए केंद्रीय जसजवल सेवा (पेंिन संरािीकरण) जनयमावली, 1981 के प्ररूप -2 में आवेिन दकया िाएगा। रटप्पण 4: आधार सं. िेना स्ट् वैजच्छक ह।ै तथाजप, यदि यह दिया िाता है, तो यह समझा िाएगा दक केवल पेंिन से संबंजधत उद्देश्य के जलए बैंक खाते से िोड़ने और यूआईडीएआई से पहचान के प्रमाणीकरण के जलए सहमजत िी गई ह।ै ख-I कुटुंब के सिस्ट्यों का जववरण [जनयम 50(15), 57, 58, 59, 60, 62, और 80 िेखें] 1. पजत/पत्नी सजहत कुटुंब के सभी सिस्ट्यों (चाह े वे कुटुंब पेंिन के जलए पात्र हों या नहीं, िो िीजवत ह)ैं के जववरण दिए िाएं जिसके अंतगित सभी बच्चों, माता-जपता/सास-ससुर और जन:िि भाई-बहन (सभी भाई और बहन) सजम्मजलत ह।ैं 2. कुटुंब के दकसी सिस्ट्य की जन:ििता या वैवाजहक प्राजस्ट्थजत में पररवतिन के जवषय में तथ्य को भी 'रटप्पणी' स्ट्तंभ में उपिर्िित दकया िाए। 3. पत्नी और पजत में न्याजयक रूप से पृथक पत्नी और पजत सजम्मजलत होंगे। क्रम नाम िन् मजतजथ आधार सं. सरकारी कमिचारी के वैवाजहक रटप्पणी सं. (दिन/मास/वष)ि साथ नातेिारी प्राजस्ट्थजत 1 2 3 4 5 6 1 2 3 4 5 6 7 8 रटप्पण: - (i) सेवा पुजस्ट्तका में िाजमल नहीं दकए गए नए नाम िोड़े िाने के मामले में, िन् मजतजथ का प्रमाण प्रस्ट् तुत करना होगा। (ii) यदि सरकारी कमिचारी और उसके पजत या पत्न ी के अजतररक् त दकसी अन् य व् यजि द्वारा प्ररूप 6-क प्रस्ट् तुत दकया िा रहा है, तो सरकारी कमिचारी द्वारा पहले से प्रस्ट् तुत कुटुंब के सिस्ट् य के जववरण को, जववरण में िोड़ा िाएगा और कुटुंब के सिस्ट् य के जववरण में कोई पररवतिन नहीं दकया िाएगा। ख. III यदि सह-प्राजधकृत दकए िाने वाला कुटुंब का सिस्ट्य मानजसक मंिता सजहत दकसी मानजसक जवकार या जन:िक् तता से ग्रस्ट्त ह ै तो संरक्षक/नामजनिेजिती का जववरण, िहां भी लागू हो : क्रम सहप्राजधकृत - संरक्षक / संरक्षक / आधार पैन नं. सरकारी मानजसक रूप िरू भाष/ पता स.ं कुटुंब के सिस्ट्य नामजनिेजिती नामजनिेजिती नं. कमिचारी से जन:िक् त मोबाइल नं . का नाम का नाम की िन्मजतजथ के साथ कुटुंब के सिस्ट्य मेल-और ई संबंध के साथ संबंध 1 2 3 रटप्पण :- (i) स्ट्थायी रूप से जन:िि बालक/बालकों/भाई-बहन और/या आजित माता-जपता के नाम पीपीओ में तभी िोड़े िाएंगे, यदि कुटुंब पेंिन के जलए कोई अन्य पात्र पूव ििावेिार नहीं ह।ै (ii) कुटुंब के दकसी अन्य सिस्ट्य के मामले में सह-प्राजधकार अमान्य हो िाएगा यदि कुटुंब का कोई अन् य सिस्ट् य सह-प्राजधकृत कुटुंब के सिस्ट्य से पहले कुटुंब पेंिन का हकिार हो िाता ह।ै प्रस्ट्ततु दकए िान ेवाले िस्ट्ताविे ों की सूची सलं ग्न ह।ै 1. िो नमूना हस्ट्ताक्षर (पृथक िीट में प्रस्ट्तुत दकए िाएं)। यदि िावाकताि अपना हस्ट्ताक्षर नहीं कर सकता तो उस े नमूना हस्ट्ताक्षर के स्ट् थान पर िस्ट्तावेि पर अपने बाएं/िाएं अंगूठे का जनिान लगाना होगा। 2. पहचान का प्रमाण। 3. पेंिनभोगी के साथ संबंध का प्रमाण। 4. िन् मजतजथ ििािने वाला अन् य प्रमाणपत्र। 5. स्ट्थायी पते के प्रमाण के साथ संरक्षक के फोटो आईडी प्रूफ की एक प्रजत। 6. संरक्षक के िो नमूना हस्ट्ताक्षर (एक अलग िीट में प्रस्ट्तुत दकए िाने चाजहए यदि कुटुंब का सिस्ट्य अवयस्ट् क ह ैया मानजसक जन:िक् तता से ग्रस्ट्त ह)ै8 THE GAZETTE OF INDIA : EXTRAORDINARY [PART II—SEC. 3(i)] 7. यदि संरक्षक हस्ट्ताक्षर नहीं कर सकता ह ै तो उसे नमूना हस्ट्ताक्षर के स्ट्थान पर िस्ट्तावेज़ पर अपने बाएं/िाएं अंगूठे आदि का जनिान लगाना होगा। 8. कुटुंब पेंिन के िावे के समथिन में आय के संबंध में अंजतम आयकर ररटनि, जिसके न होने पर एसडीएम से प्रमाण पत्र, जिसके न होने पर, आय के संबंध में कोई अन्य िस्ट्तावेि। ग. उपिान, सामान्य भजवष्य जनजध और केंद्र सरकार कमचि ारी समूह बीमा योिना के जलए सामान्य नामजनििे न प्ररूप। [केंद्रीय जसजवल सेवा (पेंिन) जनयमावली, 2021 का जनयम 46, सामान् य भजवष्य जनजध (केंद्रीय सेवाएं) जनयमावली, 1960 का जनयम 5 और केंद्र सरकार कमिचारी समूह बीमा योिना, 1980 का पैरा 19.7 िेखें] i. कोई उपिान जिसका भुगतान केंद्रीय जसजवल सेवा (पेंिन) जनयमावली, 2021 के जनयम 44 और जनयम 45 के अधीन प्राजधकृत दकया िाए, ii. कोई रकम िो मेरे सामान् य भजवष्य जनजध खाते में िमा हो iii. कोई रकम िो केंद्र सरकार कमिचारी समूह बीमा योिना, 1980 के अधीन केंद्र सरकार द्वारा संस्ट् वीकृत की िाए। लाभ का नामजनिेजिती कमिचारी/ प्रत्येक यदि नामजनिेजिती स्ट्तम्भ (2) के अधीन प्रत्येक उस व्यजि का नाम, वह प्रकार का नाम, पेंिनभोगी को अवयस्ट्क है, तो उस नामजनिेजिती की को िन्मजतजथ और पता, आकजस्ट्मकता से नातेिारी संित्त संित्त जिसके घरटत िन्मजतजथ व्यजि का नाम, कमिचारी से पूव िमृत्यु और पता दकया होने की ििा में, दकया िो स्ट्तम्भ (6) में होने पर िाने िन्मजतजथ और पता, िाने आनुकजल्पक नामजनिेिन आनुकजल्पक वाला िो अवयस्ट्क के वाला नामजनिेजिती के अजवजधमान्य अंि जनजमत्त रकम प्राप्त नामजनिेजिती का नाम, अंि अवयस्ट्क होने की ििा हो िाएगा कर सकेगा िन्मजतजथ, नातेिारी में रकम प्राप्त कर सकेगा और पता 1 2 3 4 5 6 7 8 9 उपिान सामान्य भजवष्य जनजध समूह बीमा योिना ये नामजनिेिन मेरे द्वारा पूवि में दकए गए दकन् हीं नामजनिेिनों को अजधक्रांत करेंगे। रटप्प ण:- (i)य दि प्ररूप 6-क सरकारी कमिचारी के अजतररक् त दकसी अन् य व् यजि द्वारा प्रस्ट् तुत दकया िा रहा है, तो सरकारी कमिचारी द्वारा पहले स े प्रस्ट् तुत नामजनिेिन को जववरण में िोड़ा िाएगा और नामजनिेिन में कोई पररवतिन नहीं दकया िाएगा। (ii) पूणि रकम के अंि के अंतगित नामजनिेजिती(यों)/आनुकजल्पक नामजनिेजिजत(यों) को संिेय सारी रकम आ िानी चाजहए।[भाग II—खण् ड 3(i)] भारत का रािपत्र : असाधारण 9 घ. (i) अन्य मंत्रालय/जवभाग संगठन के जलए लागू नहीं होता जलखें (ii) यदि प्ररूप दकसी सरकारी कमिचारी की ओर से भरा िा रहा ह ैतो इस भाग को काटा िाए। च. सवे ाजनवजृ त्त के पश्चात ् केंद्रीय सरकार स्ट्वास्ट्थ्य योिना के अधीन जचदकत्सा सजु वधाओं का लाभ लने े या जनयत जचदकत्सा भत्ता के जलए जवकल्प मैं जनम्नजलजखत प्रसुजवधा के जलए जवकल्प िेता हं i. मैं केंद्रीय सरकार स्ट्वास्ट्थ्य योिना क्षेत्र के भीतर जनवास करुंगा और केंद्रीय सरकार स्ट्वास्ट्थ्य योिना का लाभ लूंगा। ii. मैं केंद्रीय सरकार स्ट्वास्ट्थ्य योिना क्षेत्र के भीतर जनवास करुंगा ककंतु केंद्रीय सरकार स्ट्वास्ट्थ्य योिना का लाभ नहीं लूंगा। मैं समझता ह ंदक मैं जनयत जचदकत्सा भत्ता (एफएमए) के जलए पात्र नहीं होऊंगा। iii. मैं गैर-केंद्रीय सरकार स्ट्वास्ट्थ्य योिना क्षेत्र में जनवास करुंगा ककंतु अंतरंग रोगी जवभाग (आईपीडी) और बजहरंग रोगी जवभाग (ओपीडी) उपचार के जलए केंद्रीय सरकार स्ट्वास्ट्थ्य योिना का लाभ लूंगा। मैं जनयत जचदकत्सा भत्ता के जलए पात्र नहीं होऊंगा। iv. मैं गैर-केंद्रीय सरकार स्ट्वास्ट्थ्य योिना क्षेत्र में जनवास करुंगा ककंतु केंद्रीय सरकार स्ट्वास्ट्थ्य योिना अंििानों का संिाय करके ही आईपीडी उपचार के जलए केंद्रीय सरकार स्ट्वास्ट्थ्य योिना का लाभ लूंगा। मैं ओपीडी उपचार के जलए जनयत जचदकत्सा भत्ता का भी लाभ लूंगा। v. मैं गैर-केंद्रीय सरकार स्ट्वास्ट्थ्य योिना क्षेत्र में जनवास करुंगा और आईपीडी उपचार तथा ओपीडी उपचार िोनों के जलए केंद्रीय सरकार स्ट्वास्ट्थ्य योिना का लाभ नहीं लूंगा। मैं जनयत जचदकत्सा भत्ता का लाभ लूंगा। vi. मैं अपने पजत/पत्नी/कुटुंब सिस्ट्यों को उपलब्ध जचदकत्सा सुजवधाओं का लाभ लूंगा िो सरकार/पजब्लक सेक्टर उपक्रम/स्ट्वायत्त जनकाय का कमिचारी ह/ैपेंिनभोगी है। मैं केंद्रीय सरकार स्ट्वास्ट्थ्य योिना और जनयत जचदकत्सा भत्ता का लाभ नहीं लूंगा। vii. मैं पूव ि संगठन की जचदकत्सा सुजवधा का लाभ लूंगा। मैं केंद्रीय सरकार स्ट्वास्ट्थ्य योिना और जनयत जचदकत्सा भत्ता का लाभ नहीं लूंगा। viii मैं पूव ि संगठन की जचदकत्सा सुजवधा/ईसीएचएस सुजवधा/अन्य स्ट्वास्ट्थ्य सुजवधा का लाभ लूंगा। मैं केंद्रीय सरकार स्ट्वास्ट्थ्य योिना और जनयत जचदकत्सा भत्ता का लाभ नहीं लूंगा। छ. वचनबधं (जनयम 57, 58, 60, 63, और 80 िेखें) सेवा में, िाखा प्रबंधक .................... .................... मेरे अनुरोध पर, आपके पास मेरे खाते में प्रत्येक मास िमा करके मुझे पेंिन/कुटुंब पेंिन का संिाय करने के जलए सहमत होने पर, मैं, अधोहस्ट्ताक्षरी, दकसी ऐसी रकम जिसका मैं हकिार नहीं ह ं या कोई ऐसी रकम िो मेरे खाते में उस रकम से जिसका मैं हकिार ह ं या होगा, अजधक िमा की गई हो, को वापस करने या उसकी पूर्ति करने के जलए सहमत ह ं और इसका वचन िेता ह।ं मैं अपने और अपने वाररसों, उत्तराजधकारी, जनष्पािकों और प्रिासकों से आबद्ध करने के जलए इस योिना के तहत मेरी पेंिन/कुटुंब पेंिन को मेरे खाते में िमा करने में बैंक को हुई दकसी भी हाजन से और उसके जवरुद्ध क्षजतपर्ू ति करने के जलए और उसे बैंक (भाग क के क्रम सं. 3 में उजल्लजखत) को तत्काल भुगतान करने के जलए और बैंक को मेरे उि खाते या बैंक के कब्िे में मेरे दकसी अन्य खाते/िमा राजि को डेजबट करके िेय राजि की वसूली के जलए अपररवतिनीय रूप से अजधकृत करने के जलए सहमत ह ंऔर इसका वचन िेता ह।ं 2. पजत/पत्नी की िन्म की तारीख .......................ह ैऔर उसके पहचान का जचन्ह...... है।[भाग II—खण् ड 3(i)] भारत का रािपत्र : असाधारण 11 ि. सत्य ापन मैं प्रमाजणत करता ह ंदक इस प्ररूप के लबंि ुक से छ तक मेरे द्वारा दिए गए जववरण और घोषणाएं मेरे सवोत् तम ज्ञान के अनुसार सत् य ह।ै दिनांक: स्ट् थान: सरकारी कमचि ारी/इस प्ररूप को प्रस्ट्त ुत करन ेके जलए पररवार के प्राजधकृत सिस्ट्य (नाम सजहत) के हस्ट्त ाक्षर सरकारी सेवक के ऊपर दिए गए ब्यौरे सत्याजपत दकए गए और सही पाए गए। दिनांक: स्ट् थान: कायािलयाध्यक्ष के हस्ट्ताक्षर प्ररूप 6-क के साथ सलं ग्न दकए िान ेवाल ेिस्ट्ताविे ों की सचू ी 1. िो नमूना हस्ट्ताक्षर (पृथक िीट में प्रस्ट्तुत दकए िाएं)। यदि िावाकताि अपना हस्ट्ताक्षर नहीं कर सकता तो उसे नमूना हस्ट्ताक्षर के स्ट् थान पर िस्ट्तावेि पर अपने बाएं/िाएं अंगूठे का जनिान लगाना होगा। 2. पजत/ पत्नी के साथ संयुि फोटोग्राफ की तीन प्रजतयां अथवा यदि पजत या पत्नी के साथ संयुि फोटोग्राफ प्रस्ट्तुत करना संभव नहीं ह ै तो स्ट्वयं के और पजत या पत्नी के ऐसे पररवार के सिस्ट्य या सिस्ट्यों के फोटोग्राफ की तीन प्रजतयों के साथ अलग-अलग फोटोग्राफ जिनके नाम सह-प्राजधकृत कुटुंब पेंिन भोगी के रूप में पेंिन संिाय आिेि में िाजमल दकए िाने ह।ैं (फोटोग्राफ कायािलयाध्यक्ष द्वारा अनुप्रमाजणत दकए िाएं) 3. अनुभव (वैकजल्पक) के अधीन ब्यौरे प्रस्ट्तुत करने के जलए प्ररूप 4. पैन काडि की प्रजत रटप्पण :मूल जनयम भारत के रािपत्र, असाधारण, भाग II, खंड 3, उपखंड (i), में सा.का.जन. सं. 868(अ), तारीख 20 दिसंबर, 2021 द्वारा प्रकाजित दकए गए थे और तत् पश् चात् सा.का.जन.सं. 770(अ) तारीख 7 अक् टूबर, 2022 द्वारा संिोजधत दकए गए थे।12 THE GAZETTE OF INDIA : EXTRAORDINARY [PART II—SEC. 3(i)] MINISTRY OF PERSONNEL, PUBLIC GRIEVANCES AND PENSIONS (Department of Pension and Pensioners’ Welfare) NOTIFICATION New Delhi, the 16th July, 2024 G.S.R. 410(E).—In exercise of the powers conferred by the proviso to article 309, and clause (5) of article 148 of the Constitution and after consultation with the Comptroller and Auditor- General of India in relation to persons serving in the Indian Audit and Accounts Department, the President hereby makes the following rules further to amend the Central Civil Services (Pension) Rules, 2021, namely: - 1. Short title and commencement.-(1) These rules may be called the Central Civil Services (Pension) Amendment Rules, 2024. (2) They shall come into force after one hundred twenty days from the date of their publication in the Official Gazette. 2. In the Central Civil Services (Pension) Rules, 2021, (hereinafter called the said rules), in rule 50, in sub-rule (15), for clause (e), the following clause shall be substituted, namely:- “(e) The Government servant shall submit the up to date details of the family again in Form 6-A at the time of retirement from Government service.”. 3. In the said rules, in rule 53,- (a) for sub-rule (1), the following sub-rule shall be substituted, namely:- “(1) Unless otherwise exempted by a general or special order of the Government, the pension case of a Government servant shall be processed through online mode (Bhavishya/e-HRMS) and the retiring officials, who are on e-HRMS, shall submit Form 6-A through e-HRMS (only superannuation cases). Retiring Government servants, who are not on e-HRMS and for cases other than superannuation (i.e. Voluntary retirement, Premature retirement, Permanent absorption in State Government/Public Sector Undertaking/Autonomous Body, Invalidation, Compulsory retirement and Dismissal/Removal from service), shall submit Form 6-A through Bhavishya.”; (b) in sub-rule (2), in clauses (a) and (b), for “Bhavishya”, the following shall be substituted, namely:- “online mode (Bhavishya/e-HRMS)”. 4. In the said rules, in rule 57, - (a) in sub-rule (1), in clause (c) for sub-clause (iii), the following sub-clause shall be substituted, namely:- “(iii) advise the retiring Government servant to submit Form 6-A”; (b) in sub-rule (2), for clause (a), the following clauses hall be substituted, namely:- “(a) The Government servant shall submit duly completed Form 6-A to the Head of the Office, not later than six months prior to his date of retirement.”; (c) in sub-rule (3), in clauses (a) and (b), for “Form 4 and Form 6”, shall respectively be substituted “Form 6-A”; (d) for the proviso, the following proviso shall be substituted, namely:- “Provided that where the said form is submitted by the spouse or any other member of the family, the Government servant shall not be entitled to the benefit of commutation of a percentage of pension until he himself subsequently applies for such commutation in accordance with the Central Civil Services (Commutation of Pension) Rules, 1981.” 5. In the said rules, in rule 58, - (a) for sub-rule (1), the following sub-rule shall be substituted, namely:- “(1) A Government servant, who is retiring or has retired for reasons other than superannuation, shall submit Form 6-A to the Head of Office, immediately after the competent authority has approved such retirement or the retirement has become effective, as the case may be.”; (b) in sub-rule (2), in clauses (a) and (b), for “Form 4 and Form 6”, “Form 6-A” shall respectively be substituted.[भाग II—खण् ड 3(i)] भारत का रािपत्र : असाधारण 13 (c) for the proviso, the following proviso shall be substituted, namely:- “Provided that where the said form is submitted by the spouse or any other member of the family, the Government servant shall not be entitled to the benefit of commutation of pension until he himself subsequently applies for such commutation in accordance with the Central Civil Services (Commutation of Pension) Rules, 1981.” 6. In the said rules, in rule 59, - (a) in sub-rule (1), for “Form 4 and Form 6”, “Form 6-A” shall respectively be substituted; (b) in sub-rule (2), for “rule 58 have”, “rule 58 has” shall be substituted. 7. In the said rules, in rule 60, - (a) in sub-rule (1), in clause (i), for “copies of Form 4, Form 6 and an undertaking to the Bank in Format 9”, “Form 6-A” shall be substituted; (b) in sub-rule (3), for “a copy of each of the Forms”, “copies of Form and Format” shall be substituted; (c) in sub-rule (4), for “Form 4 and Form 6”, “Form 6-A” shall be substituted. 8. In the said rules, in rule 62, in sub-rule (6), in clause (a) for “Form 4 and Form 6”, “Form 6-A” shall be substituted. 9. In the said rules, in rule 63, in sub-rule (4), in clauses (a) and (b), for “Format 9”, “Form 6-A” shall be substituted. 10. In the said rules, in rule 80, in sub-rule (5), (i) for the proviso, the following proviso shall be substituted, namely:- “Provided that if there is no member of the family eligible to receive family pension on death of Government servant, a member of the family in whose favour a nomination was made by the Government servant for payment of gratuity, shall be allowed to submit Form 6-A in place of Form 10 and the said member of the family shall indicate, the details of his or her Bank Account in Form 6-A.”; (ii) for clause (c), the following clause shall be substituted, namely:- “(c) The Head of Office shall send Form 4, Form 10, Format 9 and Format 13 (if applicable) or Form 6- A, as the case may be, with Form 7 and a forwarding letter in Format 10, to the Accounts Officer for authorisation of pension, retirement gratuity and family pension, if applicable.”; 11. In the said rules, in Form 4, in para 1, for “along with Form 6”, “in Form 6-A” shall be substituted. 12. In the said rules, for “Form 6”, “Form 6-A” shall be substituted. 13. In the said rules, in Form 7, under the sub-heading “ FORM – 7 CHECK LIST FOR HEAD OF OFFICE FOR TIMELY PROCESSING OF RETIREMENT DUES”, item 9 and item 10 shall be omitted. 14. In the said rules, in Format 10, under the sub-heading “Enclosures”,- (a) for item 2, the following item clause shall be substituted, namely:- “2. Form 6-A or Form 10” (b) for item 3, the following item clause shall be substituted, namely:- “3. In case of Form 10, details of family in Form 4 and undertaking to the Bank in Format 9” (c) for item 4, the following item clause shall be substituted, namely:- “4. F. Option for availing Medical Facilities under Central Government Health Scheme or Fixed Medical Allowance after retirement. I opt the following facility i. I will be residing in a CGHS area and would be availing CGHS facility ii. I will be residing in a CGHS area but would not be availing CGHS facility. I understand that I will not be eligible for Fixed Medical Allowance (FMA) iii. I will be residing in non-CGHS area but would be availing CGHS facility for In-patient Department (IPD) and Out-patient Department (OPD) treatment. I will not be eligible for FMA iv. I will be residing in a non-CGHS area but would be availing CGHS facility for IPD treatment only by payment of CGHS contributions. I will also avail FMA for OPD treatment. v. I will be residing in a non-CGHS area and would not be availing CGHS facility for both IPD treatment and OPD treatment. I will avail FMA. vi.
