Memo No.45/CS.IIOP/2012-1, Dated 11-01-2012. 4.From the Civil Surgeon Specialist, Civil Dispensary, A.P., Secretariat Hyd., Ltr.No 175/CDS/2012 Dated.21.01.2012. ***** ORDER:- Sri Syed Fareed, Office Subordinate, Consumer Affairs, Food and Civil Supplies Department, in his application 2nd read above has requested to reimburse an amount of Rs.7,775/- towards medical expenses incurred by him towards the treatment of his Son for “Compound hyperopic astigmatism in both Eyes and optic atrophy, sensory estropia in the left eye” at L.V. Prasad Eye Institute, Banjara Hills, Hyderabad, on 16-12-2011. 2. The Civil Surgeon Specialist, Civil Dispensary, A.P., Secretariat, Hyderabad, n its report 4th read above after scrutiny of the medical bills of the individual in terms of G.O. 1st read above has recommended for reimbursement of Rs.7,555/-(Rupees Seven thousand Five hundred Fifty Five only) 3. The Government, after careful examination, hereby accord sanction for an amount of Rs,7,555/-(Rupees Seven thousand Five hundred Fifty Five only) to Sri Syed Fareed, Office Subordinate, Consumer Affairs, Food and Civil Supplies Department, towards reimbursement of the medical expenses incurred by him towards the treatment of his Son for “Compound hyperopic astigmatism in both Eyes and optic atrophy, sensory estropia in the left eye” at L.V. Prasad Eye Institute, Banjara Hills, Hyderabad, on 16-12-2011. 4. The amount sanctioned in para 3 above shall be debited to the Head of account”3451-Secretariat Economic Services – MH 090 – Secretariat S.H(04) Food & Civil Supplies Department – 017 Medical reimbursement”.5. The Consumer Affairs, Food and Civil Supplies (OP) Department, are requested to draw the amount sanctioned in para 3 above and disburse the same individual. 6.
