Amrutha Valli, SO., (Retd.), M.W. Dept., Application dated 01-12-2011. 3) Govt.Memo.7124/O.P/A2/11-1, dated 14-12-2011. 4) From the Civil Surgeon Specialist, Govt. Civil Dispensary, Sectt., Lr.Rc.No.2400/CDS/12, dated 17-01-2012. ******* ORDER: Sanction is hereby accorded for an amount of Rs.4,252/- (Rupees four thousand two hundred and fifty two only) to Smt. A. Amrutha Valli, Section Officer (Retd.), Minorities Welfare Department towards reimbursement of the amount spent for purchase of medicines towards the treatment i.e., CAD-Unstable Angina (CABG- 2003) DM-2, HTN., Hypothyroidism, O A Both Knees Disease and undergoing treatment at Yashoda Hospital, Malakpet, Hyd., during the period from September and October, 2011 in terms of the orders issued in the reference 1st read above 2. The amount sanctioned in Para-(1) above shall be debited to the Head of Account β€œ2251 - Social Services - 090 - Secretariat – S.H. (15) - Minorities Welfare Department - 010 - Salaries - 017 - Medical Reimbursement”. 3. Certified that the Medical bills have been scrutinized by the Civil Surgeon Specialist, Govt. Civil Dispensary, A.P. Secretariat, Hyd, vide reference 4th read above. 4. The Minorities Welfare (OP-Claims) Department shall draw and disburse the amount to the individual. 5.
