No.258/CDS/ 12, Dated. 31-01-2012 from the Civil Surgeon, Secretariat Dispensary, Hyderabad * * * ORDER: In the reference first read above, Dr Priyadarshi Dash, IAS, Special Chief Secretary to Govt., ( Retd), A.H, D.D.&F Deptt. has applied for reimbursement of medical bills, along with the Essentiality Certificate for an amount of Rs.3,602/- towards treatement of his wife for Hypertension & DM at Govt Civil Dispensory, Secretariat. In the reference third read above, the Civil Surgeon, Secretariat Dispensary, Hyderabad has scrutinized the bills & allowed an amount of Rs.3,602/- in terms of the orders issued in G.O.Ms.No.74, HM& FW (K1) Dept., dated 15-3-2005 .2) Sanction is hereby accorded for an amount of Rs.3,602/-(Rupees three thousand six hundred and two only) to Dr Priyadarshi Dash, IAS , Special Chief Secretary to Govt. (Retd) towards the reimbursement of medical expenditure incurred by him for the treatment of his wife for Hypertension & DM at Govt Civil Dispensory, Secretariat on 16-12-2011 to thrre months period in terms of the orders issued in G.O.MS NO.447 G.A (SC.X) Department. Dated.11-07-2008 3) The expenditure sanctioned in para 2 above shall be debited to the Head of Account “3451-Sectt.Economic Services – 90 – Secretariat –19 – AH DD & F Deptt. – 010 – Salaries – 017 – Medical Reimbursement”. 4) The Animal Husbandry, Dairy Development & Fisheries (OP. Claims) Department shall draw the sanctioned amount in para 2 above and arrange for payment to Dr Priyadarshi Dash, IAS, Spl Chief Secretary to Govt.
