KISHAN REDDY) (a) to (d): A statement is laid on the Table of the House. ***** 1STATEMENT REFERRED TO IN REPLY TO PARTS (a) TO (d) OF THE RAJYA SABHA STARRED QUESTION NO. 99 TO BE ANSWERED ON 09.02.2026 ASKED BY SHRI SANDOSH KUMAR P, HON’BLE MP REGARDING ‘ACCIDENTS IN COAL MINES’. (b): The causes of each fatal/ serious accident, year-wise, are attached at Annexure-I. The following corrective measures have been taken to prevent the occurrence of such accidents:- 1. Introduction of Root Cause Analysis (RCA) based Accident Investigations mechanism. 2. Formulation and implementation of Hazard Identification and Risk Assessment (HIRA) based Safety Management Plans (SMPs), Standard Operating Procedures (SOPs), Code of Practices (COPs) and Scientific Study-based Strata Control and Monitoring Plans (SCAMPs) 3. Mine Safety Audit 4. Safety Awareness Programs: Video clips and animation films focusing on safety. 5. Open Cast Mine Safety Measures: a. Roll out of blast-free mining with eco-friendly surface miners b. Formulation and implementation of mine-specific Transport Rules c. Simulator-based training for HEMM operators d. Competency Assessment for HEMM Operators through Competency Board e. Dumpers and other HEMM with advanced safety features 6. Development of National Coal Mine Safety Reporting Portal (NCMR) and Centralised Safety Information System (CSIS) for consolidated data collection, analysis, and monitoring. (c) & (d): Yes, the compensation is provided to the family in case of injuries and deaths by the respective coal companies as per the provisions of the Employees' Compensation Act, 1923 which, inter-alia, provides for payment of compensation to employees and their dependents in case of injury and accident arising out of and in the course of employment and resulting in disablement or death. The Act is implemented by the State/ Union Territories. Coal India Limited provides statutory compensation under the Employees’ Compensation Act, 1923, and a special relief/ ex-gratia payment of ₹25 lakhs. In addition, the family of a deceased departmental employee is entitled to ₹1,56,250 under the Life Cover Scheme (LCS) and an ex- gratia amount of ₹90,000 as per National Coal Wage Agreement (NCWA-XI). Employment is also provided to an eligible dependent of the deceased departmental employee or monetary compensation in case of refusal of employment. Further, insurance coverage under the Corporate Salary Package Account is provided @ ₹1 crore for departmental employees and @ ₹40 lakhs for contractual employees. 2Annexure-I ANNEXURE REFERRED TO IN REPLY TO PART (b) OF RAJYA SABHA STARRED QUESTION NO. 99 FOR 09.02.2026 BY SHRI SANDOSH KUMAR P, REGARDING ACCIDENTS IN COAL MINES. The cause for each accident, year-wise, during 2022-25: Sl. mine, it hit the rear end of stationary tipper waiting at a three way 1 junction causing serious bodily injuries to the empty tipper driver, which turned into fatal after six days while treatment in a hospital.” 03-01-2025 Khottadih Colliery 1 While a person approached the edge of a strata bunker of an underground coal 2 mine, he fell into the bunker, which was 52 meters deep, sustaining serious bodily and succumbed on the spot. 05-01-2025 Kankanee Colliery 1 While an excavator of boom height 7m was loading coal from the toe of a 29m high bench of a fiery opencast coal mine, a large quantity of hot fiery strata slid from the sides of high bench causing breakage of window glass pane of the excavator and some portion of 3 hot strata fell into pool of water which was being used to extinguish burning coal resulting formation of steam which entered inside the operator’s cabin inflicting serious burn injury to the excavator operator to which he succumbed after a period of one day in the hospital. 09-01-2025 Basantimata Dahibari 1 While a 60 tonne dumper was clumping overburden in an opencast coal mine, 4 some boulders fell on the body of a person standing on the rear side of the dumper inflicting him serious bodily 3injuries which turned into fatal on the spot. 04-02-2025 Jhanjra Project Colliery 1 While a fitter was examining the snub drum of a running belt conveyor installed at drive head unit in a belowground coal mine, his left hand 5 was entangled between snub drum and belt conveyor causing severing of his left hand from shoulder which turned into fatal at the spot”. 12-02-2025 Kujama OCP 1 While an Electrician was pulling a 185Sq.mm XLPE cable towards a 11KV vacuum Circuit Breaker in the surface Sub-station of an open cast coal mine, suddenly he slipped and 6 inadvertently came in contact with the live 11KV busbar of above mentioned switchgear, received electric shock and burn injuries to which he succumbed on the way to hospital”. 20-02-2025 NCPH Colliery R-6 Mine 1 While LHD operator, operating his LHD in a gallery to reach to face for lifting of blasted coal from the face, front portion roof bolt, loosely affixed in w- strap in the roof for reducing the sagging in electric trailing cable of 7 UDM Machine crossing across the gallery in the roof tied with installed roof bolts, pierced/penetrated in his neck below left ear to which he succumbed to his injury while shifting to the Hospital" 06-03-2025 Chhattarpur Mine-1 3 While three persons entered under a freshly exposed unsupported roof in development face of a belowground coal mine, suddenly a layer of sandstone roof measuring about 6.36m 8 in length, 5.5m in width and 0.2m to 0.3m in thickness, dislodged from a height of 2.5m and fell over them 4inflicting grievous injuries to which all three of them succumbed instantly.” 12-03-2025 Balram(Kalinga) OCP 1 While four(4) workers, standing on trestles(TR14, TR15) trying to align a pre-fabricated structure (conveyor gallery) of about 11T weight on the two trestles (TR14, TR15) i.e., matching of holes of the conveyor gallery with the holes of trestles with help of Crane in the process of alignment, two slings out of four slings tied with conveyor 9 gallery came out from the hook of the crane, the conveyor gallery slide down and hit the trestle (TR 15) towards the SILO side, fell down on the floor along with trestle(TR15), injuring one worker fatally, one co-worker escaped with minor injury and remaining two co- workers standing on trestle(TR14) escaped unhurt.” 19-03-2025 Karma Open cast Project 1 While a water tanker was going into the mine, stopped suddenly due to engine block burst and subsequently it rolled down the gradient and hit the highwall side and toppled on the haul road 10 causing serious injury to the operator, whose health condition worsened due to prevailing health issues and post- surgery complications, to which he succumbed after ten days while at the hospital.” 21-03-2025 AMLOHRI OCP 1 While a workman was greasing the shovel bucket at overburden bench of an opencast coal mine, the service van for greasing, while being reversed towards the shovel bucket pressed the 11 workmen between the rear portion of the service van and the shovel bucket inflicting serious injuries, to which he succumbed while being treated at hospital”. 518-04-2025 Gare Palma IV/2 & 3 2 While a group of three persons, sitting Opencast Coal Mine inside the truck mounted blasting shelter, kept at 165m from the blasting site, were conducting deep hole blasting in an opencast coal mine, a blasting projectile measuring about 12 0.3m x 0.25m x 0.21m hit and penetrated the blasting shelter inflicting fatal injuries to one of them on the spot, one person succumbed to his injuries after two days while undergoing treatment and rest one person was seriously injured.” 22-04-2025 Churcha Mine (RO) 1 While a person was engaged for handling of trailing cable of stuck Twin Bolter which was being pulled by a battery hauler by use of a steel chain, in 13 an underground coal mine, suddenly the Twin Bolter got released from Jam and hit the worker on his abdomen, causing serious bodily injuries, to which he succumbed about 02 hrs later. 27-04-2025 DULANGA COAL 1 While a minibus was transporting MINING PROJECT workmen from a parking yard to the Attendance Room at the end of their shift in an opencast coal mine, it’s driver momentarily lost his attentiveness and control, causing the bus to overturn to left side as its right- 14 side tyres climbed onto the road divider, during which one of the workmen riding therein got thrown out of its door and caught between the hard road surface and body of the toppled bus, receiving serious bodily injuries and was subsequently declared brought dead on being shifted to hospital. Accident Ramnagore Colliery 1 While a dumper loaded with Occurred on overburden material was heading 16-02-2025 towards internal dump in an opencast 15 Turned Into coal mine, the dumper swayed on its Fatal on 04- left side, moved over overburden heap 05-2025 lying outside the haul road and tilted on its right resulting in serious bodily 6injuries to the dumper operator which turned into fatal after 77 days during his treatment in the hospital. 05-05-2025 GODAVARIKHANI 1 “While a person was driving the water COAL MINE (No.2&2A tanker on a haul road of an opencast and No.5) coal mine, suddenly he lost control, fell down on the ground and was run over 16 by right rear tyre of the same water tanker, resulted in to crushed head injury to which he succumbed instantaneously”. 10-05-2025 Bharatpur OCP 1 While a contractual worker deputed to supervise coal-transportation was standing at a slightly elevated place about 50m away from a coal face in an opencast mine, a pay loader being reversed for parking at the end of 3rd 17 shift hit him, when in an attempt to save himself, he jumped and fell on boulders lying nearby, in which he sustained internal injuries, lost consciousness and later declared brought dead on being taken to hospital." 12-05-2025 Giddi A OCP 1 “While a 60 Tonne dumper was being positioned near a shovel for taking load at an overburden face of an opencast coal mine, an overman, who was 18 present in the vicinity for supervision, was run over by front left wheel of the dumper receiving serious bodily injuries and later declared brought dead on being taken to hospital.” 18-06-2025 Gevra OC Mine 1 “While a person was engaged in maintenance work on 6.6 kV power cable joints at Coal Handling Plant in an Opencast Coal Mine, he inadvertently came in contact with 19 energized 6.6 kV power cable temporary joint of an adjacent belt conveyor, received an electric shock, to which he succumbed on the way to hospital.” 719-06-2025 Parej East Open Cast 1 While an electric shovel was doing face Project preparation for loading of coal at a coal bench of an opencast mine, the shovel operator, perplexed by the noise of a side fall from another nearby bench at about 30 m away, suddenly swung the 20 shovel, in which an overman present nearby for supervision was hit by bucket of the shovel, receiving serious head injuries, to which he succumbed to his injuries on the way to the hospital" 29-06-2025 Kedla Underground 1 While an Electric Helper and a General Project Mazdoor were working on a pole in the 11 KV overhead line by positioning themselves at a height of 6.5 Meters without safety belt and 8 Meters with safety belt respectively and without 21 following proper shutdown procedures, they got electrocuted, which resulted in fall of the Electric Helper from the height of 6.5 Meters leading to fatal injuries and the General Mazdoor received serious burn injuries.” 10-07-2025 AMALGAMATED 1 While a dumper was negotiating a blind SENDRA BANSJORA right turn on a haul road of an open cast NICHITPUR COLLIERY coal mine, it collided with another 22 stationary dumper and, in that course, hit a person inflicting him serious bodily injuries in which he succumbed instantaneously. 23-07-2025 Gevra OC Mine 1 While a drill helper was crossing a haul road, near an overburden face of an opencast coal mine, suddenly he was 23 hit by an empty truck and received serious bodily injuries to which he succumbed subsequently. 27-07-2025 NCPH Colliery R-6 Mine 1 While a crew was engaged in replacing surface conveyor belt of an 24 underground coal mine, the belt was being pulled by inch starting of motor from the tail end and a workman (tyndal) was manually positioning the 8belt near the drive drum (out of sight from tail end), suddenly he got pulled by the moving conveyor and entrapped between the belt & drive drum causing mutilation of body parts to which he succumbed instantly on the spot.” 01-08-2025 KALYANKHANI -5 1 While a person was trying to board into INCLINE MINE the operator cabin of a Side Discharge Loader (SDL) after cleaning the blasted coal pieces stuck beneath the SDL’s rear end frame at the corner of a slice of the depillaring panel in a below 25 ground coal mine, suddenly a piece of shaly coal measuring about 1.1 m (length) x 0.25 m (width) x 0.25 m (thick) parted from the side of the rib and fell over him resulted into serious bodily injury to which he succumbed on the way to hospital 20-08-2025 Gare Palma IV/7 coal 1 On date 20/08/2025 around 5.45 am an mine incident occurred at parking area of Gare Palma IV/7 Coal Mine within the mine leasehold area. Outside dispatch 26 truck driver Shri Vijay Kumar Kol, (Age - 38 yrs)of M/s Shri Nivas Road lines Transport, Raipur was found dead condition near his truck number CG-04 NV 6898 at parking area of the mine. 20-08-2025 Shyamsundarpur Colliery 1 While a gallery was being developed by Continuous Miner in a belowground coal mine, it got connected with an old waterlogged working situated in the 27 same seam, causing inrush of water resulting in drowning of Continuous Miner operator due to which he succumbed on the spot.” 05-09-2025 Amalgamted Keshalpur 7 While a service van carrying six West Mudidih persons including driver were travelling on the haul road of an 28 opencast coal mine, a part of highwall strata measuring size of about 50m (length) X 24m (width) X 10m (height) situated immediately above the haul 9road collapsed, pushed the vehicle off the road into water accumulated 70m below the haul road resulting fatal injuries to all six persons on the spot and an outsider present in the vicinity also received fatal injury. 08-09-2025 INTEGRATED 1 While a contractual security guard was LAKHANPUR washing his hands at a small pool of BELPAHAR LILARI water near a water tanker’s filling point OCP at an opencast coal mine, a truck which was being reversed just after it’s 29 windshield cleaned hit him, causing him to fall backward and receiving serious head injuries due to hitting on the concrete floor, to which he succumbed on the way to hospital.” 24-09-2025 Urimari Open cast Project 1 While two workers, including a female worker, were engaged in stemming of a shot hole charged with explosives in an opencast coal mine, suddenly, the shot hole exploded pre-maturely due to hotness inside the hole, as a result of 30 which both the workers sustained severe injuries, one of them succumbed to his injuries while being transported to the hospital, and the other female co- worker survived with serious bodily injuries.” 02-10-2025 Ballarpur 3&4 Pits 1 At around 12:25 AM midnight, Shri. Hemraj Bhaurao Dhawas, Pump Khalasi, was going down by 4 No. cage. When the cage had descended to about 25-30 meters from the surface, the Banksman heard a shouting sound from the shaft. The Banksman immediately stopped and signalled to 31 raise the cage. On checking, it was found that the person (Shri. Hemraj Bhaurao Dhawas) was not present in the cage. The Onsetter at the shaft bottom confirmed that the person had jumped from the descending cage, maybe an act of suicide. He was subsequently found dead at pit bottom 1008-10-2025 Nigahi OCP 1 At about 02:20 AM on 08.10.2025 (third shift of 07.10.2025), a coal- loaded tipper (Reg. No. UP64CT4158) of M/s Maihar Cement and Pipe Industries Ltd. while tipper moving on the left side of the road between surface miner and weighbridge it crossed road diagonally breaches the berm and then 32 toppled and fell down to a lower bench while moving from the Surface Miner Bench (East Section) to Morwa Siding (SPUR-2) on the coal transport road. The driver, Shri Tarkeshwar Patel, sustained serious injuries and was shifted to NSC, Jayant, where he was declared dead by the attending doctor. 09-10-2025 Gevra OC Mine 1 At Gevra project on dated 09/10/2025 in 2nd shift, at about 6.10 PM, one coal loaded trailer No- CG 12 AS 0812 belonging to M/S-Aryan Coal Beneficiation after taking load from Old Dipka stock, the driver was going for final weighment at weigh bridge No- 10 but crossed the berm near tri- 33 junction behind west MTK. It seems that the driver of the trailer namely Shri Sanup Tigga, aged about 34 Years lost control over the trailer and hit the berm of the road. The driver might have jumped from the trailer causing severe injury in head. He was sent to NCH Gevra, where he was declared dead. 09-10-2025 Amalgameted Inder-1 On date 09.10.2025, Shri Shrikant Kamptee Deep OC Mine Ramkrushna Nitnaware was deployed as Shovel Operator on excavator no. 179 in 2nd shift in Departmental Kamptee patch of the mine. At around 8.30 pm, Shri Raviprakash, Mining 34 Sirdar observed that excavator no. 179 was marching in unplanned/uncontrolled manner & the excavator had stopped away from loading point. Further dumper loading was also stopped. When mining sirdar came close to excavator, he noticed, 11that the excavator operator, Shri Shrikant Nitnaware was not present in the excavator. Then he saw crushed body of Shri Shrikant Nitnaware on the path that excavator had travelled just now. The Operator Shri Shrikant was run over by his own machine and on the basis of the circumstantial evidences, prima facie it appears to be a case of suicide. 11-10-2025 Amlai Opencast Mine 1 Blasted OB was being dumped at Magazine patch bottom dump over existing old pump station platform/bench for levelling and strengthening purpose. At about 05:00 PM due to failure of lower deck of the platform where dumping was being carried out, sudden dump slided down/shearing occurred. In this incident, one dozer and one tipper of M/s-RKTC Infratech slided down into 35 the accumulated water. Dozer operator Shri Chandan Kushwaha, who also got involved in this incident, were immediately rescued and sent to Burhar Central Hospital Dhanpuri for treatment but tipper along with operator Shri Anil Kushwaha went drowned and dozer operator Shri Chandan Kushwaha suffered fracture in left hand one metacarpal and searching operation is still being carried out. 18-10-2025 Tara (East & West) Coal 1 While a tipper was moving on haul Mine road in reverse direction to load overburden in an opencst coal mine, an unqualified supervisor walking on the 36 haul road was run over by the tipper, sustained serious bodily injuries and was declared brought dead in the hospital. 24-10-2025 Duchichua OCP 1 On dated 24.10.2025 at about 11.00 37 pm in IIIrd shift (Night Shift), Pushpendra Singh S/o Shri Banke Lal 12Singh aged about 50 years(DOB- 12.12.1975), Designation – Mazdoor of M/s GSCO Infrastructure Pvt. Ltd.(an outsourced HOE company) was inadvertently hit by the dozer of the same HOE agency at the dumping in East OB dump of the Dudhichua mine and was fatally injured . 26-10-2025 Gopalichuck Colliery 1 A boulder suddenly rolled from the south-west highwall of the mine and 38 struck the diesel tanker, causing it to topple. As a result, three person in the diesel tanker received injuries. 04-11-2025 KDH Opencast Project 1 At approx 8pm on 04.11.2025 ,Mr Sukhdev Ganjhu who was working at the bunker construction site under M/s Madhucon Projects Ltd.,KDH Project,NK Area was coming down after completing his job slipped from 39 the stairs and fell down resulting in serious head injury and was sent to Dakra Central Hospital for initial treatment at Mahavir Manipal Hospital, Ranchi he was declared dead in emergency ward at around 10:56pm on 04.11.2025. 17-11-2025 Dudhichua OCP 1 On 17/11/2025, at around 1:30 PM, at Dudhichua CHP Crusher No. 3, Sri Ram Sajiwan Vaish, Fitter, EIS No. 26118489 (Departmental), aged 52 years was allegedly hit on forehead standing nearby, by arm of breakdown rock breaker. Clearing work of jammed crusher was going on by Back Hoe, 40 meanwhile, the boom of Back Hoe collided with boom of breakdown rock breaker. Consequently, Rock breaker arm moved and allegedly hit on forehead of the victim standing nearby. He was immediately rushed to NSC, hospital where he was declared brought dead. 1323-11-2025 Jharkhand Opencast 1 On 23.11.2025 during the General Project Shift, a departmental truck with driver were engaged for loading of pump at workshop area. The truck parked at the site for loading started moving unexpectedly in forward direction and 41 the driver tried to climb the running truck , fell on the ground causing serious injury. He was shifted to nearby hospital where it was advised after first aid to shift to other hospital for better treatment and the injured succumbed on the way. 30-11-2025 Jayant OCP 1 While fitting rollers onto the roller frame of Belt No. 1. at approximately 11:30 AM, Shri. Arvind Khairwar, Contractual helper fainted and was taken to the hospital, where he was 42 declared died. The reason of death is being enquired. Apart from other reasons, the possibility of electrocution is also being examined. Final reason will be confirmed by post mortem report. 20-12-2025 Urimari Open cast Project 2 A Fatal accident occurred at Contractual Patch of Urimari OCP, Barka-Sayal Area of M/s Central Coalfields Limited, on 20th December 2025 at about 21:45 hours (end of 2nd shift) in which two persons, namely- 1. Sri Sunil Yadav, Tipper Driver, age about 29 years 2. Sri Raju Paswan, 43 Dump-man, age about 55 years, were fatally injured due to partial bench slide on the tipper stationed for loading at toe of the lower bench. Probable Cause of accident as reported partial OB bench slide probably due to some hidden fault/ folds. Detailed enquiry is under process to ascertain the exact cause and contributing factors 1425-12-2025 Amalgamated Amlo Dhori 1 At around 11:30 am on dated Opencast Mine 25.12.2025, Himanshu Raj, ITI Fitter, contractor’s employee of M/s Minsol Limited under AADOCM, Dhori Area, CCL who was deployed at pumping station of Highwall Patch for supervision and maintenance of pumping and dewatering, he slipped down near pumping station during his 44 supervision and got head injury. He was immediately taken to the Central Hospital Dhori for treatment. After diagnosis, he has been referred to KM Memorial Hospital, Bokaro for specialized treatment. The KM Memorial Hospital, Bokaro after preliminary investigation declared him dead. While a person was proceeding to drinking water point in congested haulage roadway of a development belowground coal mine, suddenly he got skidded on trackline and fell down 45 08-01-2024 Kasipet 2 Incline 1 and hit the derailed tub existed adjacent to haulage trackline, resulted into serious bodily injury to his right leg (with fracture) and succumbed in the hospital after 17 days during medical treatment. “While a set of five empty tubs were being hauled by a direct rope haulage in a de-pillaring district of a belowground coal mine, tub attached with the rope got derailed, suddenly the second tub separated from the first tub 22/01/2024 and the set of four empty tubs were ran Godavrikhani Coal Mine 46 T/T/F/ 1 back for a distance of about 70 m and [No. 2 & 2A and No. 5] 28/01/2024 hit Sirdar, who was at the junction of endless haulage, along with trammer and other two persons, inflicting serious bodily injuries to Sirdar & others were escaped unhurt, and Sirdar succumbed to his injuries in the hospital after seven days”. 15While the contractor operator of an empty tipper, parked his tipper near the PC which was under maintenance by two contractual workers in the OB bench of an opencast coal mine, alighted his cabin with the engine in 47 29-01-2024 Krishnashila OCP 1 ‘ON’ position, suddenly the tipper rolled back on its own in uncontrolled manner, which ran over one of the worker, inflicting him serious bodily injuries to which he succumbed almost instantaneously”. While a contractual worker was removing a detached roller and broken liner pieces from beneath the moving belt conveyor in an opencast coal mine, his band got entrapped in between 48 17-02-2024 Kusmunda OC Mine 1 roller and conveyor belt and was severely pulled, causing him serious internal bodily injuries to which he succumbed while being treated at hospital. While a Battery Haulder Machine Operator after parking the Battery Hauler entered into the route of another Battery Hauler in an underground coal mine, he was hit by the Battery Hauler 49 19-02-2024 Kurja UG Colliery 1 going to unload the coal on feeder breaker on that route, inflicting serious bodily injuries to which he succumbed during treatment at the hospital after about twenty-four hours. While a worker was present in continuous miner depillaring panel and was standing in NO GO ZONE, i.e. at the corner of split Junction of pillar under extraction and CM machine was 50 06-03-2024 Churcha Mine (RO) 1 loading the RAM CAR by sweeping & cleaning the split gallery before start of slicing, suddenly a violent coal bump occurred in pillar under extraction and lumps of coal parted from the unsupported side wall, out of fear he run away hurriedly and fell on to the 16ground, inflicting serious bodily injury to head, legs and forearm injuries, he was referred to super specialty hospital after primary treatment at the regional hospital of the mine, where he was declared brought dead.” While a bunch of W - straps weighing about 1.25 tons was being lifted using a nylon round sling by a mobile crane in a store yard, the impaired sling snapped near the crane hookand the KalyanKhani No. 1 Incline 51 16-03-2024 1 bunch of W-straps fellon a person from Mine a height of about 1.5m, who was assisting the rigging loads, due to which he sustained serious bodily injuries and succumbed on the way to hospital. While a crew of five contract workers engaged in repair work of headgear structure were cleaning the bell plate platform sheets located at height of twenty meters, the bell plate platform along with guide rope clamp mounting girder failed from one side resulting in 52 02-04-2024 Chinakuri Mine No 1 2 fall of two persons. One of them fell into the shaft bottom along with safety belt and received fatal injury while other hanged with safety belt but was inflicted serious bodily injury due to platform and rope to which he scummed after a few minutes. While a person from top of box cut was observing the status of dewatering operation being carried out at the bottom of box cut excavated for an underground coal mine, fell down from Urtan North U/G Coal 53 10-04-2024 1 a height of about 11.5m and also a Mine stone boulder measuring 1.0m x 0.75m x 0.60m size dislodged from about 2.5m height fell on him causing serious bodily injury to which he succumbed during treatment at the hospital” 17While a hired light motor vehicle driven by contractor’s driver with 05 employees was passing from front right corner of a stationary Dumper waiting in queue on the haul road for taking load in an opencast coal mine, it suddenly moved forward and run over 54 23-04-2024 Nigahi OCP 1 the light motor vehicle by front and then rear right side wheels, inflicting serious bodily injuries to 04 persons of which one person succumbed to injuries during treatment in hospital after one and half hour when other two escaped with minor injuries". “While truck driver was exiting in hurry with loaded truck from coal stock yard, on the haul road having nearly 1 in 90 gradient and 13 m to 18 m wide, a security guard whilst talking over Integrated Lakhanpur mobile phone at his non-designated 55 24-04-2024 1 Belpahar Lilari OCP work place, inadvertently entered in close proximity of running truck was hit and partially run-over by its front right side wheel received serious bodily injuries, to which he succumbed on the way to hospital” While, a trammer was coming out from the cage pushing an empty coal tub at the pit-bottom of a belowground coal mine, stone pieces having size 37 cm X 15 cm X 15 cm, 25 cm X 10cm X 56 10-05-2024 Parasea Colliery 1 10cm fallen from sides of shaft over protective roofing and cage canopy, got broken into pieces and hit on the head and back of neck of the trammer inflicting serious bodily injurie to him, due to which he succumbed instantly. “While an excavator operator was cutting a drain at the bottom Amod(G-19 Extn.) Lignite overburden bench, of an opencast 57 10-05-2024 1 mine lignite mine, suddenly the in-pit overburden dump, back filled against an in-situ barrier of overburden benches, slid down, burying the 18excavator along with its operator, resulting into fatality of the operator instantly”. “While a person was performing maintenance work on a Dozer on an overburden dump, he accidentally drove off on it and the Dozer slid down Kabribad Reorganised 58 12-05-2024 1 along the slope of the dump to a depth Mine of about 25 m during which he received grievous bodily injuries and succumbed to the injuries while undergoing treatment in a hospital.” While a trammer was approaching to pull the empty tubs from the winder cage at Pit bottom in a belowground coal mine, a spring of temporary Pit bottom buffer was detached from mounting girder and carried away by 59 15-05-2025 JAMBAD COLLIERY 1 the ascending winding cage bottom channel and fell down from 151 meters height and bounced back thereby hitting the victim in his forehead causing serious injury to which he succumbed almost instantaneously”. While a contractual welder and helper were engaged for repairing and installation of firefighting water hydrant line over bottom deck plate of a belt conveyor of coal handling plant of an opencast coal mine, suddenly the 60 22-05-2024 Dudhichua OCP 2 support angle of the deck plate dislodged to which they both fell down from a height of about 17 m on concrete floor and received serious bodily injury to which they succumbed almost instantaneously on the spot”. While a security guard deployed for guarding the machinery in an internal Sasti Expansion Opencast overburden dumping area where 61 24-05-2024 1 Mine dozing and leveling work was going on was inadvertently hit and dozed by the dozer, went missing and his dead body was found buried under the slope of the 19freshly dozed overburden dump at about 3.0m down from the crest of the dump”. While an operator of a load haul dump (LHD) machine was standing on dip side of an already loaded and parked load haul dump machine near the side wall of a drift-gallery in an underground coal mine, the said load Godavarikhani No.11 62 30-05-2024 1 haul dump machine suddenly started Incline rolling down due to which he was caught and pressed in between side of the pillar and rear end of the load haul dump machine, thus received serious bodily injuries, to which he succumbed on the spot” While a tipper driver after exiting from weighbridge parked his loaded tipper by the right side of coal transportation road and was engaged in fixing of tarpaulin by standing at the edge of the tipper’s coal-laden dump box in an 63 11-06-2024 Kulda Opencast Project 1 opencast coal mine, he lost his balance and fell down from a height of about 3m and was run over by rear right side wheels of another loaded tipper passing nearby, receiving serious bodily injuries to which he succumbed on the spot instantaneously.” While a light motor vehicle driven by contractor’s driver with 05 contractor’s employees was travelling along the haul road in an opencast coal mine it suddenly hit and run over by the dumper’s rear right-side wheels near 64 28-06-2024 Jayant OCP 1 the curved portion of the haul road at about 12m from the junction, inflicting serious bodily injuries to 03 persons of which one person succumbed instantaneously at the spot when other two escaped with minor injuries and one person unhurt". 20While a contractual workman engaged for examination and maintenance of belt conveyor at top bench in an opencast lignite (coal) mine was crossing the stationary belt conveyor, 65 08-07-2024 Neyveli Lignite Mine-II 1 suddenly the belt started and he was carried over it to the tail end, where his head was hit and stuck under v-wiper, inflicting serious bodily injuries to which he succumbed on the spot”. While a fitter along with Three General Mazdoors was repairing the water pipe line in a trench of about 5.2 m depth made to expose the pipe line buried under loose overburden materials in an opencast coal mine, suddenly, the side of the trench measuring about 2.0 m Ramagundam Opencast-ii 66 17-07-2024 2 (long) x 5.2 m (height) x 0.5 to 2.0 m Mine (thickness) parted from the side and slid down, due to which fitter and one of the General Mazdoors got buried under the same which resulted into fatal injuries to them while two others escaped with reportable and minor injury respectively”. While during a heavy rain, six persons on a workplace, were staying in a rest shelter located near the bank of a drain in an opencast coal mine, silt & debris carried along with the flow of water choked the hume - pipes, on the upstream side, resulting in spread of overflow water around the rest shelter; the six persons while trying to escape 67 27-07-2024 Kusmunda OC Mine 1 from the shelter, one of them slipped, lost control and was carried away alongwith the water into sump, at a distance of about 100m, filled with water upto a depth of about 10 m, resulting in his death, who was later recovered after approximately 17 hours and the other five persons escaped unhurt”. 21While a contractual workman deployed for signaling discharge boom operator of bucket wheel excavator for positioning the boom over transfer NEYVELI LIGNITE feeder in an opencast lignite (coal) 68 06-08-2024 1 MINE NO.-1A mine, was sleeping on roadway near the transfer feeder, he was run-over by a marching dozer, inflicting serious bodily injuries to which he succumbed instantaneously.” While an Assistant Mechanic, squatting on the left hand side (LHS) traction chain, was priming to remove air lock from a metallic fuel pipe provided in between Fuel Filter and Fuel Pump of a Dozer parked in the haul road of an opencast coal mine, 69 13-08-2024 New Majri UG to OC Mine 1 another Assistant Mechanic sitting on the operator’s seat, started the dozer in forward gear on which dozer moved forward, dislodging the Assistant Mechanic squatting on the traction chain and was run over by the LHS traction chain, inflicting serious bodily injuries that proved fatal instantly. While 05 persons including contractual employees were engaged for installation of a dewatering pump with base frame on floating pontoon by use of an excavator & two rope slings in an opencast coal mine, suddenly one of 70 21-08-2024 BLOCK B 1 the rope slings got slipped, resulted tilting of pump set at one side and then fell on one of the contractual employees ,who received serious bodily injury to which he succumbed instantaneously on the spot while others escaped unhurt” 29-09-2024 Chhal Opencast Mine “While a group of 06 Contractual work persons were manually lifting the steel 71 1 tubular pole of length about 13 mtrs. to place into a pit with the help of rope, near by the Chhal OCM Substation, all of a sudden the steel tubular pole got 22slipped and fell down on one of his Contractual work person neck and got serious injuries. Then he was shifted to hospital, where the doctors declared him brought dead.” 30-09-2024 Nigahi OCP While a welder was engaged in welding of monorail support at the roof of a transfer house of under construction coal handling plant of an opencast coal mine and when he gets down for refreshment, inadvertently fell down 72 1 into the depth of 15.7 mtr on concrete floor from the opening/gap at the side of the discharge chute at same platform, resulted serious bodily injuries to which he succumbed on the way to hospital”. 03-10-2024 Gevra OCMine On dated 03/10/2024 at Gevra Project, emulsion pump truck of M/s Shri Krishna Explosive and Accessories bearing registration no. CG 04JA-8450 was engaged for charging of blast hole at patch M/s SMASL patch (NIT-522). After charging of explosive, the said pump truck was returning back at about 03:25PM.On its way from Narabodh to 223 junction, when it was about 200M 73 1 away from 223 junction it become uncontrolled and went over the right- side berm and toppled opposite to driver side. During this the Technical Service Engineer of M/s Special blast, Shri Gorelal Patel S/o Manglu Ram Patel, aged about 54years got serious injury on the back. He was immediately shifted to NC Hospital Gevra, where he was declared dead. 06/10/2024 Orient Colliery Mine While dressing roof at immediate out- TF bye junction of a development gallery 27/11/2024 after blasting in an underground coal 74 1 mine, a block of shaly coal measuring about 0.9m x 0.6m x 0.15m lying between two roof bolts, suddenly detached from height of 2.6m and hit 23the chest of dresser after slipping along dressing rod, subsequently dresser fell down on his back on blasted coal inflicting serious internal bodily injuries to which he succumbed after undergoing treatment for 52 days at hospital”. 07-10-2024 GANGARAMCHACK & While unloading of explosives from an GANGARAMCHACK- explosive van (loaded with booster BHADULIA COAL explosives, electric and electronic MINE detonators, and shock tubes) parked near the site scheduled for blasting at an overburden bench of an opencast coal mine, and primer preparation, carrying them in bunches to the shot holes, and lowering of primer charge into a hole were going on simultaneously, suddenly, a strong 75 8 blast occurred in the explosive van, resulting in shock waves, air overpressure and ejection of splinters from the damaged aluminium sheeting of the van, which in turn initiated to blast all the primer charges, to which all eight persons engaged in the above operations died on the spot; and one of the three persons present nearby received serious bodily injuries and two received reportable injuries. 15-10-2024 Gevra OC Mine While a contractual worker was engaged in cleaning of bottom liner plates inside the silo by standing on ladder in an opencast mine, suddenly a mild steel (MS) plate of size 1m x 1m of 4 mm thickness slid inside along the 76 1 silo wall from the top of silo opening from a height of about 30 meter and hit the worker on his lower back portion, inflicted serious internal injuries to him to which he was succumbed while undergoing treatment at hospital.” 2419-10-2024 Rajnager OC Mine “While a person taking shelter along with other three persons in a Bolero vehicle on the top of overburden bench within the danger zone from the place of blasting of an opencast coal mine, a piece of fly rock ejected from the blasting site from the bottom 77 1 overburden bench, hit a part of roof of the Bolero vehicle and subsequently the roof of the vehicle bent down and hit one person sitting in Bolero vehicle causing serious bodily injury to which he succumbed during shifting to the hospital” 18-11-2024 Selected Dhori Opencast While a person driving an empty mine Dumper down the gradient in an open cast coal mine, continuously for more 78 1 than the stipulated time, lost control and hit OB Dump in which he received serious bodily injury to which he succumbed later." 24-11-2024 Magadh OCP While a Dozer Operator, in drunken state, was marching the dozer from coal stock yard to workshop at the end of his shift, he lost his balance at the middle of the haul road and caused the dozer to 79 1 fell down from the haul road in to the quarry bed at about 25 m, below the haul road, to which he received grievous bodily injuries and succumbed to the injuries on the spot” 03-12-2024 Godavarikhani No.7 LEP “While a manager of a belowground coal mine entered inside a stowing bunker installed on the surface to supervise the operation of dislodging stuck up material there, loose material 80 1 on which he was standing caved in causing him buried under debris and later when he was rescued and sent to the hospital, where he was declared brought dead.” 2504-12-2024 Jhiria UG Mine While two workmen were engaged for dressing of a newly blasted roof at gallery junction in depillaring panel of an underground coal mine, one of them fell down during dressing of roof and the other attempted to rescue him, 81 2 suddenly a lump/stone measuring 1.4m x 1.1m x 0.1m size parted from the roof from a height of 3.05m and fell over them inflicting serious bodily injury to both of them to which they succumbed during treatment at the hospital” 19-12-2024 Parkashamkhani Opencast While a loaded dumper was being Mine reversed on a dump for unloading overburden material in an opencast coal mine, it rolled back and fell down 82 1 to immediate bottom bench to a depth of 29 m, inflicting serious bodily injuries to the dumper operator, who succumbed on the way to the hospital.” “While dressing of roof at junction having a loose coal layer of about 0.3m thick in depillaring panel of an underground coal mine was being supervised by Mining Sirdar standing beneath the junction received injuries 83 19-10-2023 Bahula Colliery 1 due to fall of the dressed layer measuring about 5.4m x 4.4m x 0.15 - 0.3m fallen from a height of 3.0 m also inflicting serious bodily injuries to two dressers in which mining sirdar succumbed to injuries. While, a face crew was escaping from high pressure hydraulic fluid coming out from a valve got broken during operation which was kept on the Bridge Stage Loader and being used for taping 84 09-10-2023 Jhanjra Project Colliery 1 hydraulic fluid to erect of OC Prop at Gate roadway in Long-wall Panel, fallen down on the floor containing big lumps of coal inflicting injuries to his left side face and front-temporal reason due to which he succumbed later on. 26While an empty dumper was being parked in the parking yard of an opencast coal mine, another dumper 85 16-01-2023 Gourangdi (A) Colliery 2 operator, who was present in the parking yard after parking his dumper run over by it and succumbed almost instantly. While a person deployed at pit bottom of pit no.4 left his post/designated place and transgressed to the exposed space 86 01-06-2023 Nimcha (R) Colliery 1 of the empty tubs cage line, he got pressed under the empty cage to which he received serious bodily injuries and succumbed to his injuries. While empty tubs were being coupled to each other at the top of haulage plane of haulage roadway of an underground coal mine, it got derailed and hit two persons coming out through haulage 87 09-07-2023 Tilaboni Colliery 1 roadway resulting serious bodily injuries to them due to which one person succumbed to injury during treatment in the hospital after 3 hours and other person received serious bodily injury. While a person was inside the cabin of a Pay Loader near the toe of the of a coal bench, rock mass weighing about one tonne got dislodged from the side Amalgamated Konar and of overlying overburden bench and fell 88 25-06-2023 1 Khasmahal OCP from a height of about 70 m on the Pay Loader inflicting serious bodily injuries to the Operator to which he succumbed in a hospital after eight days. While a person trying to open the left door of operator's cabin to deal with fire occurred due to leakage of Diesel and Short Circuit at dispensing pump of 89 20-05-2023 Ena Colliery 1 a diesel bowser standing at haul road of an open cast coal mine, received serious burn injuries to which he succumbed on the spot. 27While a loaded dumper was moving towards the overburden dump yard in an opencast coal mine, it hit and run 90 15-08-2023 Kujama Colliery 1 over a light motor vehicle (LMV) resulting crushing of the LMV and two persons inside the LMV were fatally injured. While a person entered in the HEMM parking area from where Dumpers were leaving for operation in an open Amalgamated Muraidih 91 13-09-2023 1 cast mine, suddenly he was hit/run over Phularitand Colliery by reversing Dumper inflicting serious bodily injuries to which he succumbed immediately. While a person was pouring water onto the shovel machine to douse its fire by the hose pipe of fire-tender in an open cast coal mine, suddenly water evaporated from the its hot surface 92 22-03-2023 Sendra bansjora 1 forming hot steam cloud which out spread in the air and steam puff touched his body inflicting burn injury on his body to which he succumbed in the hospital after time of about 20 hours. While a person was attempting to collect a plastic jar, being used to collect water from the discontinued waterlogged quarry of a coal mine for priming of pump, installed at this quarry edge, which had fallen into the 93 17-04-2023 Bhowra(South) 1 water and floating, he went deep into the water & while swimming back towards the edge he failed to swim further & drowned and his floating body was recovered from the waterlogged quarry after about 13 hours of the accident. While a tipper with its raised tipping body was being unloaded near the edge 94 25-12-2023 Bhowra(South) 1 of the overburden dump yard of an opencast coal mine, it fell down from the dump to a depth of about 8m and caught fire, inflicting serious bodily 28injuries to the tipper operator to which he succumbed in the hospital after about two and half hours. “While a person, riding a motorcycle on a haul road was on his way to the surface, a portion of overburden material measuring about 60 m in length, 8 m in height and 13 m in width got dislodged from the overlying super jacent strata, slid down the gradient, fell on the haul road and buried him, resulting in his death.” As per evidences adduced before me, the cause may be concluded as: Had (i) the workings in the northern side of the mine, overlying the haul road where the accident occurred been kept benched with heights not exceeding 3 m in overburden consisting of alluvium soil or other similar ground and the width thereof not less than three times the height of the bench and in case of overburden of rock formation, with heights not exceeding the digging 95 01-10-2023 GOVINDPUR PH-II OCP 1 height or reach of the excavation machine in use and the width thereof not less than the height of the bench, or the width of the widest machine plying on the bench plus two meters or three times the width of the dumper, if dumpers ply on the bench, whichever was more, as required under Regulation 106(4) and Regulation 106(5) of the Coal Mines Regulations, 2017; and (ii) a Scientific Study been made in the mine for determining the method of working, ultimate pit slope, monitoring of slope stability and the mine workings been made accordingly and the slope stability of the workings been monitored as required under Regulation 106(2) of the Coal Mines Regulations, 2017 read with DGMS (Tech.) Circular No. 02 of 2020 and 03 of 2020; this accident could have been averted. This accident is categorized as 29Mining-Statistical as accident occurred within the mine, as the person was employed in the mine, who was on duty and the accident occurred during mining operation as per DGMS (Tech.) Instruction No. 01 of 2017 Dhanbad, dated 04.12.2017. While a contractual supervisor, supervising tippers movement from the OB face standing at corner of four way junction, approach road to non- working upper bench and tried to stop 96 11-10-2023 Jagannath Colliery 1 an empty tipper entering the upper bench for parking at undesignated parking area slipped and got hit by empty tipper, fell down and succumbed to his injuries immediately. It is concluded that the fatal accident had occurred due to the following reasons: " While the boom cylinder of an excavator in a private coal mine was being removed during maintenance with the help of a crane & web sling, the boom cylinder came out 97 21-01-2023 West Bokaro Qr. AB 1 instantaneously to swing abruptly. as such it hit the ladder (working platform) kept for the purpose and fell on the person who tried to hold it causing him serious head injury to which he succumbed on the way to hospital." 30While a contractual Tipper was going towards the mine after unloading coal at SILO bunker in an opencast coal mine, the operator lost control of the Tipper negotiating the curve of the road 98 14-03-2023 Bharatpur OCP 1 due to over speeding and hit an abandoned building adjacent to the road causing serious injuries to him to which he succumbed almost on the spot. While a contractual Tipper operator got down the tipper at the working coal face of an opencast coal mine, the tipper rolled down the gradient and the operator attempting to board the 99 09-03-2023 Garjanbahal OCP 3 moving tipper caught between tippers when it collided with another tipper parked aside causing serious bodily injury to which he succumbed on the way to hospital. While a 40 T Crane was marching from the mine towards the surface workshop in an opencast coal mine, the crane rolled down the gradient and hit the 100 27-10-2023 Topa RO OCP(A) Mine 1 berm & toppled on the haul road and the operator trapped below the crane causing serious injury to which he succumbed almost on the spot. While driving a loaded water tanker from water loading point to workshop for scheduled maintenance, he stopped water tanker on haul road having mild gradient ( lin * 22 ) in an opencast coal 101 18-07-2023 Lingaraj OCP 1 mine, climbed down by stair case situated at front side of water tanker, fell down on ground, mean while water tanker moved forward and rolled over him and he succumbed to his injuries immediately. While a Weigh Bridge In-Charge was returning to weigh bridge from coal 102 27-09-2023 Magadh OCP 1 stock of an opencast coal mine on two- wheeler by coal transportation road 31talking on mobile phone, he was hit and ran over by rear right wheel of an empty contractor's tipper entering a coal stock yard causing serious bodily injuries to which he succumbed on the way to hospital. While a contractor worker was unwinding a jumper wire of a conductor on a double pole structure of 103 24-07-2023 Lakhanpur OCP 1 3.3kV feeder of an opencast coal mine, suddenly the power was charged electrocuting him to which he succumbed on the way to hospital. While a hired Bolero vehicle driven by untrained and unauthorized driver with six employees was overtaking a 100T Dumper on the haul road approaching towards a overburden loading face in 104 11-04-2023 Garjanbahal OCP 1 an opencast coal mine, it was hit, toppled and dragged for about 6m by the Dumper, inflicting fatal injuries to two persons and serious bodily injuries to four persons. While a contractor tipper driver was driving an empty tipper from dump yard to the working face in an opencast coal mine, the tipper collided head-to- tail with another empty tipper waiting 105 13-10-2023 BLOCK B 1 at the junction of haul roads, causing serious bodily injuries to driver to which he succumbed on the way from dispensary to hospital after about one and half hours". While contractual workers engaged for painting of pole of 33 KV overhead line feeder in an opencast coal mine, one of the worker mistakenly climbed onto a pole of other parallel live 33 KV 106 17-04-2023 JAYANT OCP 1 overhead line and came in contact with a live conductor, got electrocuted, causing severe burn injuries, instantly fell onto the ground and succumbed almost instantaneously 32While a bunch of 05 MS pipes of 8" diameter and 22' length was being unloaded from the truck with the help of wire sling and crane, suddenly an unfastened pipe slipped out from the loop from side edge of the body of the 107 06-08-2023 Amadand Opencast Mine 1 truck and hit a Foreman standing nearby, who was supervising and also helping in pipe unloading operation, causing serious bodily injuries to which he succumbed on the way to the hospital. While a contractor worker deployed as shift incharge was walking on a haul road of an opencast coal mine, a piece of stone measuring about 1.1m x 0.75m 108 22-05-2023 Dudhichua OCP 1 x0.35m fell from a loaded moving tipper from a height of about 3.2m inflicting serious injuries at his right shoulder to which he succumbed on the way to hospital. While a dozer operator was dozing for preparation of drill face on the overburden bench of an opencast coal Ramagundam Opencast -iii 109 28-08-2023 1 mine, drill operator approaching the Mine dozing area got hit and dragged by the dozer resulted in to fatal injury to him instantaneously. While a continuous miner operator along with a contractual supervisor went into an un- supported development gallery of an underground coal mine to rectify breakdown of the continuous miner, suddenly, a piece of Godavarikhani No.11 110 17-08-2023 1 shaly coal measuring about 1.2 Incline m(long)* 0 - 0.6m (wide)* 0.2m (thickness) parted from the roof and fell over them, resulted into fatal injury to the continuous miner operator and minor injuries to the contractual supervisor. 33While a track mounted electro hydraulic drill machine was being marched in a belowground coal mine, a high pressure hydraulic hose, dislodged from fatigued female swivel end fitting Adriyala Longwall Project 111 25-07-2023 1 of modified hydraulic system, resulting mine the hose to whiplash, thereby hitting a person standing in its close vicinity, inflicting serious bodily injuries to him, to which he succumbed on the way to hospital.
