Maruthi Kumari, P.S (Retd), General Administration Department towards reimbursement of Medical Expenses incurred for treatment of self for “ RVD, Gastroenteritis, UTI ” as in patient during period from 09.02.2011 to 14.02.2011. at Kamineni Hospital, L.B Nagar, Hyderabad as found admissible by the Civil Surgeon Specialist, Civil Dispensary, Secretariat, Hyderabad in the reference 4th read above. The Essentiality Certificates and the vouchers have been submitted by the individual. 2. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Reimbursement” and the same has not been sanctioned previously. 3.
