Govt.Memo.No.6747/OP-V/2012-1, G.A.(OP-V)Deptt., Dated:28.02.2012. 5. From the Civil Surgeon Specialist, Sectt., Hyd., Lr.No.494/CDS/12, Dt:02-03-2012. <<>> O R D E R: In the circumstances represented in the reference 3rd read above and in terms of the orders issued in the reference 1st and 2nd read above, sanction is hereby accorded for payment of a sum of Rs.10,000/- (Rupees ten thousand only) to Sri Mohsin Ahmed, Office Subordinate, General Administration Department, towards reimbursement of Medical Expenses incurred for self for “CHRONIC PERIODONTITIS” as in patient during the period from 02-02-2012 to 24-02-2012 at Smiline Dental Hospital, Hyderabad in terms of the orders issued in the reference 1st and 2nd read above as found admissible by the Civil Surgeon Specialist, Govt. Civil Dispensary, Secretariat, Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. Certified that, this is the first time Dental treatment of the individual and the same is recorded in the Service Register of the Individual. 2. The expenditure shall be debited to: “2052 – Secretariat General Services – MH (090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. 3. The General Administration (Claims-E) Department are requested to draw and disburse the amount to the individual. 4. This order does not require the concurrence of Finance Department as per rules in vogue. 5. This order is available on Internet and can be accessed at address http://www.ap.gov.in/goir”.
