M.G.Madhavi, Section Officer, General Administration Department towards reimbursement of medical expenses to her mother for “Cateract Surgery for left eye” as in patient on 27.01.2012 at Maxivision Laser Centre, Hyderabad as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Secretariat, Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. 2. Certified that Smt. M.G.Santha is fully dependent mother of the employee and a certificate to this effect is also enclosed. 3. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. 4.
