Department towards reimbursement of his medical expenses for “Cateract Surgery for right eye” as in patient on 18.02.2012 at Maxivision Laser Centre, Hyderabad as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Secretariat, Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. 2.
