Ramanatha Rao, P.S to C.S, General Administration Department towards reimbursement of Medical Expenses incurred for treatment of his son for “Fracture of upper front tooth” as In patient during period from 08.01.2012 to 17.01.2012. at Dilsuknagar Super Specialty Dental Hospital, Dilsuknagar, Hyderabad as found admissible by the Civil Surgeon Specialist, Civil Dispensary, Secretariat, Hyderabad in the reference 5th read above. The Essentiality Certificates and the vouchers have been submitted by the individual. 2.
