Civil Dispensary, Lr.Rc.No.492/CDS/12, Dt.28-02-2012. **** O R D E R: In terms of the orders issued in ref. 1st read above, sanction is hereby accorded for payment of a sum of Rs.13,016/- (Rupees Thirteen thousand sixteen only) to Sri U. Adikeshavulu, Record Assistant, General Administration Department towards reimbursement of medical expenses incurred for treatment of his mother for “Hypothyroidism & Calculi” from 08/02/2012 to 3 months, as recommended by the Civil Surgeon, Secretariat in the ref. 4th cited. 2. The expenditure shall be debited to “2052 - Secretariat General Services - MH 090 Secretariat – SH (04) General Administration Department - (010) Salaries – (017) Medical Reimbursement”. The same amount has not been sanctioned previously. 3. The General Administration(Claims.E)Department shall draw and disburse the amount to Sri U. Adikeshavulu, Record Assistant, General Administration Department. 4. This order does not require the concurrence of Finance Department. 5.
