From the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad, Rc.No.312/CDS/2012, dated 28.02.2012. <<>> O R D E R: In the circumstances represented in the reference 3rd read above and in terms of the orders issued in the references 1st and 2nd read above sanction is hereby accorded for payment of a sum of Rs.3,507/- (Rupees three thousand five hundred and seven only) to Sri Tuljaram, Section Officer(Retd.), General Administration Department towards the re- imbursement of his medical expenses for “HTN & HYPOTHYROID” as out patient during the period from Nov, 2011 to Jan.2012 at Civil Dispensary, A.P.Secretariat, Hyderabad as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. 2. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. 3. The General Administration (Claims-A) Department are requested to draw and disburse the amount to the individual to his A/c. No.110310011001271, Andhra Bank, Secretaraiat Branch, Hyderabad. 4.
