Padmavathi Devi for “Collid Cyst in Brain” as out patient during the period from 18.9.2011 to 17.02.2012 at Krishna Institute of Medical Sciences, Hyderabad as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. 2.
