From the Civil Surgeon Specialist, Civil Dispensary, Secretariat, Hyderabad, RC No.2468/CDS/Hyd/2012,Dt.21-1-2012. oOo O R D E R:- Sanction is hereby accorded for payment of an amount of Rs.10,000/- (Rupees ten thousand only) to Smt R.Kasturi, Assistant Secretary to Government, Agriculture and Cooperation Department towards reimbursement of medical expenses incurred by her toward her son’s dental treatment for “Root Canal Treatment” during the period from 18-11-2011 to 30-11-2011, at Smiline Dental Hospital, Punjagutta, Hyderabad. 2. The amount sanctioned above shall be debited to the Head of Account “3451-Secretariat Economic Services--090--Secretariat--018--Agriculture & Cooperation Department--010-Salaries—017- Medical Reimbursement”. 3. The Agriculture and Cooperation (Claims) Department are requested to draw and disburse an amount of Rs.10,000/- (Rupees ten thousand only) to the individual. 4.
