From the Civil Surgeon Specialist Lr.RC.No.265/CDS/2012, dt.2.3.2012 <<>> O R D E R: In the circumstances represented in the reference 2nd read above and in terms of the orders issued in the reference 1st read above, sanction is hereby accorded for payment of a sum of Rs.14000/- (Rupees fourteen thousand only) to Sri Sri. C. Narasmham, Joint Secretary to Government (Retd.), General Administration Department, towards reimbursement of Medical Expenses incurred for his wife treatment for “Cataract operation of left eye” at Akira Eye hospital, Rajahmundary as in patient during the period from 22.12.2011 to 24.12.2011. The Essentiality Certificate and the vouchers have been submitted by the retired Officer. The amount of Rs.14,000/- now sanctioned has not been sanctioned and drawn earlier. 2. The expenditure shall be debited to: 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. 3. The General Administration (Claims-A) Department are requested to draw and disburse the amount to the above retired Officer. The bank account number of the retired officer is S.B A/C No. 12941001010102 , Andhra Bank, Vanasthalipuram Branch, Hyderabad 4. This order does not require the concurrence of Finance Department. 5.
