Nanda Kumar, Assistant Secretary to Government(Retd.,), Health, Medical & Family Welfare Department while enclosing original medical bills, has requested for reimbursement of an amount of Rs.4,800/- incurred towards treatment to self for “Glucoma in Eyes” at Maxivision Hospital, Begumpet, Hyderabad as out patient. 2. In the letter 4th read above, the Civil Dispensary, Sectt., Hyderabad has scrutinized the bills and recommended an amount of Rs.4,800/- as follows: Total amount claimed … Rs.4,800-00 Admissible amount under consideration … Rs.4,800-00 ----------------- Net admissible amount Rs.4,800-00 (Rupees Four thousand eight hundred only) 3. Government after careful examination of the matter hereby accord sanction for an amount of Rs.4,800 (Rupees Four thousand eight hundred only) to Sri T. Nanda Kumar, Assistant Secretary to Government(Retd.,), Health, Medical & Family Welfare Department towards treatment to self for “Glucoma in Eyes” at Maxivision Hospital, Begumpet, Hyderabad on 12-11-2011 which was recognized by Government of Andhra Pradesh. 4. The expenditure sanctioned in para (3) above shall be debited to “2251 Secretariat Social Services – M.H.-090 – Secretariat – SH (060) Health, Medical & Family Welfare Department 010-Salaries 017, Medical Reimbursement – Voted”. 5.
