K.Kanakavalli, Asst., Secy to Govt., (Retd), Revenue Department, Appl, Dt: 04.01.2012. 3. Govt.Memo.No.375/OP.I (1)/2012-1 Revenue (OP.I) Deptt., Dated: 06.01.2012. 4. From the Civil Surgeon Specialist, Civil Dispensary, A.P. Secretariat, Hyderabad, Lr.RC.No.104/CDS/ 12, dt.28.01.2012. *********** O R D E R : Smt. K.Kanakavalli, Asst. Secy to Govt., (Retd), Revenue Department, in his application 2nd read above has stated that, she has taken treatment at Civil Dispensary, AP Secretariat, Hyderabad, during the period from November 2011 to January 2012 and she was incurred and expenditure of Rs.8,497/-. She has furnished all relevant papers and requested the Government to reimburse the said amount to him. 2. In the reference 3rd read above, the Government have sent the application and medical bills to Civil Surgeon Specialist, Civil Dispensary, A.P. Secretariat, Hyderabad with a request to scrutinize the medical bills and indicate the admissible amount for reimbursement to the Individual. 3. The Civil Surgeon Specialist, Civil Dispensary, A.P. Secretariat, Hyderabad in his letter 4th read above, has stated that the medical bills Smt. K.Kanakavalli, Asst., Secy to Govt.,(Retd), Revenue Department, towards herself and for her husband’s treatment during the period from November 2011 to January 2012 at Civil Dispensary, AP Secretariat, Hyderabad have been scrutinized in terms of APIMA Rules,1972 read with G.O.Ms.No. 107, LAW (La& J-SC-F) Deptt., dated: 14.07.2006 and offered the following remarks:- Total Amount Claimed Rs.8,497.00 Inadmissible Amount Rs. 342.00 __________ Gross Admissible amount Rs. 8,155.00 Amount limited to ceiling/package --- rates Net Admissible Amount Rs. 8,155.00 4. Government after careful examination of the matter, hereby accord sanction for payment of an amount of Rs.8,155/- (Rupees Eight Thousand One Hundred and Fifty Five only) to Smt. K.Kanakavalli, Asst., Secy. to Govt. (Retd), Revenue Department, towards reimbursement of medical expenses incurred to for herself and for her husband’s treatment during the period from November 2011 to January 2012 at Civil Dispensary, AP Secretariat, Hyderabad as recommended by Civil Surgeon Specialist, Civil Dispensary, A.P. Secretariat, Hyderabad. ( P.T.O.):: 2 :: 5. The expenditure sanctioned in para 4 above, shall be debited to “2052 – Secretariat General Services – 090 – Secretariat – 09 – Revenue Department – 010 – Salaries; 017- Medical Reimbursement.” 6.
