From Sri Mukthar Ahmed, Record Assistant (Reted), Revenue Department Application dated.19.09 .2011. 3. Govt.Memo.No.51120/OP.II/201, Revenue Dept., dt.26.12.2011. 4. from the Civil Surgeon Specialist, Govt. Civil Dispensary, A.P. Secretariat, Hyderabad Lr.Rc.No.2474/CDS/2012 Dt.19.-01-2012. -oOo- ORDER:- Sri Mukthar Ahmed, Record Assistant (Retd), Revenue Department in his application 2nd read above has stated that he has taken treatment for LV dysfunction at Care Hospital, Nampally, Hyderabad during the period from 5-05-2011 to 22-10-2011 and incurred an amount of Rs.9,074/-.He has furnished all relevant bills and certificates and request the Government to reimburse the said amount. 2. In the reference 3rd read above the Government sent the application and medical bills to the Civil Surgeon specialist, Civil Dispensary, A.P. Secretariat, Hyderabad with a request to scrutinize the medical bills and indicate the admissible amount for reimbursement to the individual. 3. The Civil Surgeon specialist, Civil Dispensary, A.P. Secretariat, Hyderabad in his letter 4th read above has stated that the medical bills of Sri Mukthar Ahmed, Record Assistant (Reted), Revenue Department towards his treatment for LV dysfunction with Care Hospital, Nampally at Hyderabad for the period from from 5-05-2011 to 22-10-2011 months has been scrutinized in terms of APIMA Rules, 1972 read with G.O.Ms.No.74,HM&FW(K1)Dept.,dt.15.3.2005 and offered the following remarks:- 1. Total amount claimed …… Rs. 9074=00 2. Less: Inadmissible amount ... Rs. 00=00 ---------------------- 3. Gross Admissible amount……….. Rs. 9074=00 4. Less 10% cut on admissible amount in terms of G.O.Ms.No.74, HM & FW (k1) Dept., dt.15.3.2005 Rs. 00 =00 ---------------------- 5. Net admissible amount ………. Rs. 9074=00 ---------------------- P.T.O//2// 4 . Government after careful examination of the matter hereby accord sanction for payment of Rs.9,074/- (Rupees Nine thousand and Seventy four only) to Sri Mukthar Ahmed, Record Assistant (Retd), Department towards the reimbursement of medical expenses incurred for his treatment for LV dysfunction at Care Hospital, Nampally, Hyderabad for the period from 5-05-2011 to 22-10-2011 months as recommended by Civil Surgeon Specialist, Civil Dispensary, A.P. Secretariat, Hyderabad. 5. The expenditure sanctioned in para (1) above shall be debited to “2052- Secretariat General Services -090- Secretariat -09- Revenue Department – 010 – Salaries,017-Medical Reimbursement”, 6. The Revenue (Claims) Department are requested to draw and disburse the amount to the individual. 7. This order does not require the concurrence of Finance Department as per rules in force.
