Read the following:-
1. G.O.Rt.No.1748 HM&FW (B2) Department, dated.19-10-2011.
2. Representation of Dr.A.Amaravathi Yadav, DAS UPHC Husnabad,
Karimnagar district dated: 15.11.2011.
3. From the Director of Public Health & Family Welfare, A.P., Hyderabad
Letter No.2003/E6.F/2009, dated.04-01-2012.

O R D E R:
In the G.O.1st read above, Government have extended the deputation of
Dr.A.Amaravathi Yadav, Dental Assistant Surgeon at UPHC, Malkajgiri, Hyderabad till
31-12-2011 in relaxation of G.O.Ms.No.145 HM&FW (A1) Department, dt.16-07-2009
and also directed that, after completion of deputation, she shall render her services at
UPHC, Husnabad, Karimnagar district only.
2. In the representation 2nd read above, Dr.A.Amaravathi Yadav, Dental Assistant
Surgeon has submitted for her transfer to clear vacancy posting or continuation of
deputation for (2) years and also submitted that, her marriage proposal is yet to be
finalized and her old aged parents health to be taken care and requested for her transfer
to Hyderabad or extend deputation for further one year.
3. In the letter 3rd read above, the Director of Public Health & Family Welfare, A.P.,
Hyderabad has submitted the service particulars of Dr.A.Amaravathi, Dental Assistant
Surgeon and stated that, she is on deputation to UPHC, Malkajgiri, Hyderabad and the
individual has requested for her extension of deputation at UPHC Malkajgiri, Hyderabad
for a further period of (1) year.
4. Government after careful examination of the matter hereby extend the deputation
of Dr.A.Amaravathi, Dental Assistant Surgeon at UPHC, Malkajgiri, Hyderabad till
31-12-2012 in relaxation of G.O.Ms.No.145 HM&FW (A1) Department, dt.16-07-2009
and after completion of deputation, she shall render her services at UPHC, Husnabad,
Karimnagar district only.
5. The Director of Public Health & Family Welfare, A.P., Hyderabad shall take
further action accordingly.
