Read the following:- 1. Representation of Dr. G. Shiva Prakash, D.C.H.S., Karimnagar dt.18-10-2011. 2. From the Commissioner, APVVP, Hyderabad Lr.Rc.No.2087/M1A/2010 Dated:21-11-2011. *** ORDER: In the reference 1st read above, Dr. G. Shiva Prakash, District Coordinator of Hospital Services, Karimnagar has represented that he is suffering from Hyper Tension and Diabetes and Hyper lipidemias. His wife is working at Anantapur in Private Engineering College and she is also suffering from Cardiomyopathy. His mother is aged above 80 years and residing at Madanapally, she also requires his presence due to chronic ill health. He has requested to post him as District Coordinator of Hospital Services, Hindupur, Anantapur District. 2. In the reference 2nd read above, the Commissioner, APVVP has stated that Dr. G. Shiva Prakash is working as DCHS, Karimnagar w.e.f. 20-8-2010. There is one vacancy of D.C.H.S., at Hindupur, Anantapur. He has requested the Government to issue necessary orders on the request of Dr. G. Shiva Prakash, DCHS, Karimnagar for his transfer as DCHS, Hindupur, Anantapur District. 3. After careful examination of the matter, Government hereby order to transfer Dr. G. Shiva Prakash District Coordinator of Hospital Services from Karimnagar to Hindupur, Anantapur District in the clear existing vacancy of District Coordinator of Hospital Services in relaxation of ban on transfer orders issued in G.O.Ms.No.127, Finance (DCM.III) Department, Dated:22-6-2011. 4. The individual is not entitled for Transfer Travelling Allowances (TTA) / DA, Joining time etc., as the transfer is effected on his own request. 5. The Commissioner, A.P. Vaidya Vidhana Parishad shall take necessary further action in the matter accordingly. 6.
