Nanda Kumar, Assistant Secretary to Government(Retd.,), Health, Medical & Family Welfare Department while enclosing original medical bills, has requested for reimbursement of an amount of Rs.4,367/- incurred towards treatment to self for “DM” at Civil Dispensay Secretariat, Hyderabad as out patient. 2. In the letter 4th read above, the C.S.S., Government Civil Dispensary, A.P. Secretariat, Hyderabad has scrutinized the bills and recommended an amount of Rs.3,584/- as follows: Total amount claimed … Rs.4,367-00 In admissible amount … Rs. 783-00 Admissible amount under consideration … Rs.3,584-00 --------------- Net admissible amount Rs.3,584-00 (Rupees Three thousand five hundred and eighty four only) 3. Government after careful examination of the matter hereby accord sanction for an amount of Rs.3,584/- (Rupees Three thousand five hundred and eighty four only) to Sri T. Nanda Kumar, Assistant Secretary to Government (Retd.,), Health, Medical & Family Welfare Department towards treatment to self for “DM” at Civil Dispensary Secretariat, Hyderabad from 30-06-2011 to 22-07- 2011. 4. The expenditure sanctioned in para (3) above shall be debited to “2251 Secretariat Social Services – M.H.-090 – Secretariat – SH (060) Health, Medical & Family Welfare Department 010-Salaries 017, Medical Reimbursement – Voted”. 5.
