Satyanarayana, Section Officer (Retd), Health, Medical & Family Welfare Department while enclosing original medical bills, has requested that he has incurred an amount of Rs.5,294/- towards treatment taken by him for “DM” at Civil Dispensary, A.P. Secretariat, Hyderabad during the period from 10-10-2011 to 31-01-2012 as an outpatient. 2. In the letter 4th read above, the C.S.S., Govt. Civil Dispensary, A.P. Secretariat, Hyderabad has scrutinized the bills and recommended an amount of Rs.5,294/- as follows: Total amount claimed … Rs.5,294-00 Admissible amount under consideration … Rs.5,294-00 ----------------- Net admissible amount Rs.5,294-00 (Rupees Five thousand two hundred and ninty four only) 3. Government after careful examination of the matter hereby accord sanction for an amount of Rs.5,294-00 (Rupees Five thousand two hundred and ninty four only) to Sri P. Satyanarayana, Section Officer (Retd), Health Medical & Family Welfare Department towards treatment to self for “DM” at Civil Dispensary, A.P. Secretariat, Hyderabad. 4. The expenditure sanctioned in para (3) above shall be debited to “2251 Secretariat Social Services – M.H.-090 – Secretariat – SH (060) Health, Medical & Family Welfare Department 010-Salaries 017, Medical Reimbursement – Voted”. 5.
