B.Vijaya Lakshmi, W/o late Sri B.Chandrasekhara Rao,, Additional Secretary (Retd.), General Administration Department, who expired on 25.12.2010 towards reimbursement of Medical Expenses incurred for the treatment of her husband for “ Chornic renal failure” as out patient at Apollo Hospital during the period from 1.12.2010 to 24.12.2010. The Essentiality Certificate and the vouchers have been submitted by Smt. B.Vijaya Lakshmi, W/o late Sri B.Chandrasekhara Rao,, Additional Secretary (Retd.), General Administration Department. The amount of Rs.21289/- now sanctioned has not been sanctioned and drawn earlier. Smt. B.Vijaya Lakshmi, W/o Sri B.Chandrasekhara Rao, Addl. Secy.(Retd.) has submitted Death certificate and Family members certificate. 2.
