From the Civil Dispensary Lr.Rc.No.2382/CDS/12, dt.6-1-2012. **** O R D E R: In terms of the orders issued in ref. 1st & 2nd cited, sanction is hereby accorded for payment of a sum of Rs.5,500/- (Rupees Five thousand five hundred only) to Sri Shaik Hameed Pasha, Xerox Operator, General Administration Department towards reimbursement of medical expenses incurred for dental treatment for replacement of tooth by “Metal Ceramic Crown” for his wife at American Institute of Dentistry, Ameerpet, Hyderabad during the period from 19-10-2011 to 24-11-2011, as recommended by the Civil Surgeon, Secretariat in the reference 5th cited. 2. The expenditure shall be debited to “2052 - Secretariat General Services - MH 090 Secretariat – SH (04) General Administration Department - (010) Salaries – (017) Medical Reimbursement”. The same has not been sanctioned previously. 3. The General Administration (Claims.E) Department shall draw and disburse the amount to Sri Shaik Hameed Pasha, Xerox Operator, General Administration Department. 4.
