Tajuddin, Typist – Cum – Assistant, General Administration Department towards reimbursement of medical expenses incurred for his treatment of “Depression” as out patient during the period from 17-10-2011 to 16-1-2012 at Govt. Civil Dispensary, A.P. Secretariat, Hyderabad as found admissible by the Civil Surgeon Specialist, Civil Dispensary, Secretariat, Hyderabad in the reference 5th read above. The Essentiality Certificates and the vouchers have been submitted by the individual. 2. The expenditure shall be debited to “2052 - Secretariat General Services - MH 090 Secretariat – SH (04) General Administration Department - (010) Salaries (017) Medical Reimbursement”. 3. The amount now sanctioned has not been sanctioned and drawn earlier. 4. The General Administration (Claims. A) Department shall draw and disburse the amount to Sri Mohd. Tajuddin, Typist – Cum – Assistant, General Administration Department. 5. This order does not require the concurrence of Finance Department. 6.
