As out patient, during the period from 26.09.2011 to 24.01.2012 at Govt.Civil Dispensary, A.P.Sectt., Hyderabad as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. 2. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. 3.
