(OP-V) Deptt., dated:7.01.2012. 4. From Global Hospitals, Hyd., Lr.No.-Nil-, Dt:05-12-2011. 5. From the Civil Surgeon Specialist, Govt. Civil Dispensary, A.P. Secretariat, Hyd., Lr.No.88/CDS/12, dated:27.1.2012. ***** O R D E R: In the circumstances represented in the reference 2nd read above and in terms of the orders issued in the reference 1st read above, sanction is hereby accorded for payment of a sum of Rs.3,000/- (Rupees three thousand only) to M/s Global Hospitals, Hyderabad towards reimbursement of Medical Expenses incurred for “Master Health Checkup” for Sri G.Narender Rao, Office Subordinate, General Administration Department on credit basis on 23-11-2011 as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Secretariat, Hyderabad in the reference 6th read above. The vouchers have been submitted by the Hospital. The amount now sanctioned has not been drawn and paid earlier to the individual. 2. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. 3.
