Raziuddin Shakir, Deputy Secretary to Govt., GAD, Dt:26.12.2011 3.Memo.No. 41063/OP-I/2011-1, G.A.(OP-I)Deptt., dt:29.12.2011. 4. From the Civil Surgeon Specialist Lr.RC.No.2497/CDS/2012, dt. 31-1-2012. <<>> O R D E R: In the circumstances represented in the reference 2nd read above and in terms of the orders issued in the reference 1st read above, sanction is hereby accorded for payment of a sum of Rs.25,491/- (Rupees twenty five thousand four hundred and ninty one only) to Sri Mohd. Raziuddin Shakir, Deputy Secretary to Govt., General Administration Department, towards reimbursement of Medical Expenses incurred for his mothers treatment for “ CAD – Acute AWMI with RBBB, SR, Trifascicular Block, Trop T – Positive, S/P:STK., Moderate LV Dysfunction, Hypertension” at Care Hospital, Hyderabad as in patient during the period from 5.10.2011 to 10.10.2011 The Essentiality Certificate and the vouchers have been submitted by the retired Officer. The amount of Rs.25,491/- now sanctioned has not been sanctioned and drawn earlier. 2. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Re-imbursement”. . 3. The General Administration (Claims-A) Department are requested to draw and disburse the amount to the above retired Officer. 4.
