ORDER:
In the circumstances stated in the reference 3rd read above, sanction is hereby
accorded for payment of an amount of Rs.2,628/- (Rupees Two Thousand Six Hundred
and Twenty Eight only) to Sri T.Mallesh, Driver (Retd.) towards reimbursement of
medical expenses incurred by him for the treatment of self for Diabetic (Sugar) at Govt.
Civil Dispensary, A.P.Secretariat, Hyderabad during the period from 10.10.2011 to
09.01.2012, as recommended by the Civil Surgeon Specialist, Govt. Civil Dispensary,
A.P.Secretariat, Hyderabad.
2. The expenditure shall be debited to “2052– Secretariat General Services -
MH(090) Secretariat – SH (04) General Administration Department – (010) – Salaries –
017 – Medical reimbursement.”
3. The General Administration (Claims.E) Department are requested to draw and
credit the amount sanctioned in para 1 above to the individual’s SB Account
No.52088448353, State Bank of Hyderabad, Secretariat Branch.
