Iqbal Hussain, Assistant Section Officer working on deputation as Personal Assistant to Minister for Tourism & Culture for reimbursement of medical expenses incurred for the treatment of Endometriosis for his wife at Government Civil Dispensary, Secretariat, Hyderabad from 30-01-2012 to 2 months, as out patient, as recommended by the Civil Surgeon Specialist, Government Civil Dispensary, Secretariat, Hyd. vide reference 6th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned above has not been sanctioned and drawn earlier. 2.
