Suryakantham for “Bronchial Asthma” as in patient during the period from 29.12.2011 to 31.12.2011 at Omni Hospitals Hyderabad as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad in the reference 5th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount now sanctioned has not been sanctioned and drawn earlier. 2.
