Vijaya Lakshmi, Record Assistant, G.A.D. towards reimbursement of medical expenses incurred for self for “Root Canal ” treatment during the period from 23-12-2011 to 06-01-2012 at FMS Dental Hospital, Koti, Hyderabad as found admissible by the Civil Surgeon Specialist, Civil Dispensary, Secretariat, Hyderabad in the reference 5 read above. The Essentiality Certificates and the vouchers have been submitted by the individual. 2. The expenditure shall be debited to “2052 - Secretariat General Services - MH 090 Secretariat – SH (04) General Administration Department - (010) Salaries – 017 Medical Reimbursement”. 3. The amount now sanctioned has not been sanctioned and drawn earlier. 4. The General Administration(Claims.E)Department shall draw and disburse the amount to Smt. B. Vijaya Lakshmi, Record Assistant, General Administration Department. 5. This order does not require the concurrence of Finance Department. 6.
