Civil Dispensary, Lr.Rc.No.151/CDS/12, Dt.31-1-2012. **** O R D E R: Sanction is hereby accorded for payment of a sum of Rs.40,693/- (Rupees Forty thousand six hundred ninty three only) to Sri K.Salaiah, Lift Operator G.A.D. towards reimbursement of medical expenses incurred for treatment of his wife Smt K. Anasuya for “LRTI, HTN, Hypothyroidism” treatment during the period from 11-12-2011 to 18-12-2011 at Yashoda Hospital, Malakpet, Hyderabad as found admissible by the Civil Surgeon Specialist, Civil Dispensary, Secretariat, Hyderabad in the reference 5 read above. The Essentiality Certificates and the vouchers have been submitted by the individual. 2. The expenditure shall be debited to “2052 - Secretariat General Services - MH 090 Secretariat – SH (04) General Administration Department - (010) Salaries – 017 Medical Reimbursement”. 3. The amount now sanctioned has not been sanctioned and drawn earlier. 4. The General Administration(Claims.E)Department shall draw and disburse the amount to Sri K.Salaiah, Lift Operator, General Administration Department. 5. This order does not require the concurrence of Finance Department. 6.
