From the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad letter Rc.No.110/CDS/2012, dated: 31.01.2012. ***** O R D E R: In the circumstances represented in the letter 3rd read above and in terms of the orders issued in the G.O 1st to 3rd read above, sanction is hereby accorded for payment of a sum of Rs.33,451/- (Rupees thirty three thousand four hundred and fifty one only) to Sri G.Sanyasi Rao, former Assistant Section Officer, General Administration Department (presently working as Section Officer, Revenue Department) towards reimbursement of medical expenses incurred to his mother Smt. G.Satyavathi for “Hystectomy” as in patient during the period from 06.04.2011 to 13.04.2011 at Subham Prema Hospital, Visakhapatnam as found admissible by the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P.Sectt., Hyderabad in the reference 6th read above. The Essentiality Certificate and the vouchers have been submitted by the individual. The amount of Rs.33,451/- now sanctioned has not been sanctioned and drawn earlier. 2. The expenditure shall be debited to : 2052 – Secretariat General Services – MH(090) Secretariat- SH(04) General Administration Department –(010)Salaries – (017) Medical Reimbursement / Advance”. 3. The General Administration (Claims-A) Department are requested to draw and disburse the amount to the above individual in his A/C. No. 110310025600467, Andhra Bank, Secretariat Br., Hyderabad. 4.
