(a) & (b): Health is a state subject, hence, the responsibility for improving the healthcare facilities
lies with the respective State/UT Governments. Under Pradhan Mantri-Ayushman Bharat Health
Infrastructure Mission (PM-ABHIM), for State of Uttar Pradesh, administrative approvals have been
accorded for 5 years (i.e. FY 2021-22 to 2025-2026) for an amount of Rs.5249.99 Cr. for
construction of 1670 Building-less SHCs-AAMs, operationalization of 674 Urban-AAMs,
establishment of 515 BPHUs, construction/ strengthening of 75 Integrated Public Health Labs
(IPHLs) and establishment of 74 Critical Care Blocks (CCBs) at District Hospitals/ Govt. Medical
College level.

As per the information provided by the State Government of Uttar Pradesh, the Unified
Disease Surveillance Platform (UDSP) portal is utilized for regular surveillance of malaria, dengue,
chikungunya,AES/JE, andother waterborne diseases across the state. Daily, real-time data isentered
on the UDSP portal from all Community Health Centers, Primary Health Centers, and Sub-Centers
in each district. This data is analyzed on a daily basis by the District Epidemiologist/District
SurveillanceUnit andat the state levelby the State SurveillanceUnit.
(c) & (d): Ministry of Health and Family Welfare provides technical and financial support for the
expansion of dialysis services under Pradhan Mantri National Dialysis Program based on the
requirements proposed by States/UTs, in their Programme Implementation Plans (PIPs) based on the
gap assessment such dialysis demand done by state and within their overall resource envelop.
Proposals received from State/UT are discussed during the National Programme Co-ordination
Committee Meeting (NPCC) and financial support is provided in the form of Record of Proceedings
(RoPs) approvals, as per the existing norms. The details of health facilities initiated or on-going

Under the Universal Immunization Programme (UIP), States/UTs are provided with Ice lined
Refrigerators (ILRs), Deep Freezers (DFs), Walk-in-coolers (WIC), Walk-in Freezers (WIF), Solar
refrigerators, cold box, vaccine carrier and Ice packs for proper storage and distribution of vaccines.
As per State PMNDP Monthly Report-31st Dec, 2025, in the State of Uttar Pradesh, PMNDP is
operational through PPP-mode in all the 75 districts. Day Care Centres also approved for 67 district
