केंद्रों और रधजस्रीकृत प्राइवेट स् वास् ् य सुधवधा-केंद्रों के माध् यम से रा्य सरकारों और संघ राज् य क्षेत्र प्रिासनों द्वारा
कायााध वत की जा रही ह;ै
और जबदक, स्कीम के ददिा-धनदेिों के अनुसार, पात्र क्षयरोग रोधगयों, प्राइवेट स् वास् ् य देखभाल प्रदाताओं और उपचार
सहायता प्रदाताओं (धज ह ें इसमें इसके पश्चात ् फ़ायदाग्राही कहा गया ह)ै को इस स्कीम के अधीन सिता नकद सहायता (धजसे
इसमें इसके पश्चात् ‘फ़ायदा’ कहा गया ह)ै प्रदान की जाती ह;ै और जबदक, उपयुाक्त स्कीम में भारत की संधचत धनधध से उपगत
आवती व् यय अतवाधलत ह;ै
अत:, अब आधार (धवत्तीय और अय सहाधयदकयों, प्रसुधवधाओं और सेवाओं का लधययत पररदान) अधधधनयम, 2016 (2016 का
18) (धजसे इसमें इसके पश्चात् ‘उि अधधधनयम’ कहा गया ह)ै की धारा 7 के उपबंधों के अनुसरण म ें स् वास् ् य एवं पररवार
कल् याण मंत्रालय, केंद्रीय सरकार धनम्नधलधखत को अधधसूधचत करती ह ैअथाात:्-
1. (1) इस स्कीम के अधीन लाभ प्राप् त करने के पात्र व् यधक्त स े आधार नंबर रखने का सबूत देना और आधार
अधधप्रमाणन करवाना अपेधक्षत होगा।

(2) इस स्कीम के अधीन लाभ प्राप् त करन े के इ् ुक व् यधक्त , धजसके पास आधार नम्ब र नह ह ै अथवा धजसन े आधार
नम्ब र के धलए अभी तक नामाकं न नह करवाया है, को 31/08/2017 तक आधार नामांकन हते ु आवेदन करना होगा,
परत ु वह उक्त अधधधनयम की धारा 3 के उपबंधों के अनुसार आधार प्राप् त करन े का पात्र हो और ससा व् यधक्त
आधार के धलए नामांकन करवाने के धलए दकसी आधार नामांकन केंद्र (भारतीय धवधिष्टट पहचान प्राधधकरण की
वेबसाइट www.uidai.gov.in पर सूची उपलब् ध ह)ै पर जा सकता ह।ै
(3) आधार (नामांकन और अद्दतन) धवधनयम, 2016 के धवधनयम 12 के अनुसार, स् वास् ् य एवं पररवार कल् याण
धवभाग से, जो रा्य सरकार अथवा सघं रा्य क्षेत्र प्रिासन म ें इस स्कीम के कायाा वयन के धलए उत् तरदायी ह,ै
अपेक्षा की जाती ह,ै दक वह ससे फ़ायदाग्रधहयों के धलए आधार नामांकन सुधवधाएं प्रदान करे धज होंने अभी तक
आधार के धलए नामांकन नह करवाया है, और यदद संबंधधत ब् लॉक अथवा तालुका अथवा तहसील म ें कोई आधार
नामांकन केंद्र अवधस् थत न हो, तो रा्य सरकार अथवा संघ रा्य क्षेत्र प्रिासन म ें इस स्कीम के कायाा वयन के धलए
उत्तरदायी धवभाग भारतीय धवधिष्ट पहचान प्राधधकरण के धवद्यमान रधजस्रारों के साथ समव य करके अथवा
भारतीय धवधिष्ट पहचान प्राधधकरण रधजस्रार बन कर सुधवधाजनक अवस्थ ानों पर आधार नामांकन सुधवधाएं प्रदान
करेगा;
परतु फायदाग्राधहयों को आधार नंबर समनुदेधित दकए जाने के समय तक, स्कीम के अधीन फ़ायदे ससे व् यधक्त यों को
धनम्न धलधखत पहचान-पत्र प्रस् ततु दकए जान े के अध् यधीन प्रदान दकए जाएंग,े अथाात्:
(क) (i) यदद उसने नामांकन करवा धलया हो तो उसका आधार नामांकन आईडी धस्ल प; अथवा
(ii) आधार नामांकन के धलए दकए गए उसके अनुरोध की प्रधत, जैसा दक पैरा 2 के उप-पैरा (2) में धवधनर्ददष्ट ट ह;ै
और
(ख) (i) भारत के धनवााचन आयोग द्वारा जारी मतदाता पहचान-पत्र; अथवा (ii) आयकर धवभाग द्वारा जारी स् थायी
खाता संख् या (पैन) काड;ा अथवा (iii) पासपोटा; अथवा (iv) मोटर यान अधधधनयम, 1988 (1988 का 59) के
अधीन अनुज्ञधप्त प्राधधकारी द्वारा जारी चालन अनुज्ञधप्त; अथवा (v) राजपधत्रत अधधकारी अथवा तहसीलदार द्वारा
िासकीय लैटर हडै पर जारी दकया गया फोटो पहचान प्रमाण-पत्र; अथवा (vi) डाक धवभाग द्वारा जारी पता काड ा
धजसपर नाम और फोटो हो; अथवा (vii) बैंक फोटो पासबुक, अथवा (viii) दकसान फोटो पासबुक;अथवा (ix)
महात् मा गांधी राष्टर ीय ग्रामीण रोजगार गारंटी स्कीम काडा, अथवा (x) रािन काड;ा अथवा (xi) सरकार अथवा संघ
रा्य क्षेत्र प्रिासन द्वारा जारी कमाचारी फोटो पहचान-पत्र; अथवा (xii) रा्य सरकार अथवा संघ रा्य क्षेत्र
प्रिासन द्वारा यथाधवधनर्ददष्ट ट कोई अ य दस्तावेज़:
परतु यह और दक रा्य सरकार अथवा संघ रा्य क्षत्रे प्रिासन द्वारा धवधनर्ददष्ट रूप स े पदनाधमत अधधकारी द्वारा
उि दस्तावेज़ों की उस प्रयोजन के धलय े जांच की जाएगी।
2. स्कीम के अधीन फ़ायदाग्रधहयों को सुधवधाजनक और धनबााध फायदा प्रदान करन े के धलए, स्कीम के कायाावयन के धलए
धज़म्मेदार रा्य सरकार अथवा संघ रा्य क्षेत्र प्रिासन में स्वास््य एवं पररवार कल्याण धवभाग धनम्नधलधखत सधहत सभी
आवश्यक व्यवस्था करेंग,े अथाता ्:-
(1) सरकारी स् वास् ् य सुधवधा-केंद्रों और प्राइवेट प्रत् याधयत स्वास््य सुधवधा-केद्रों के माध् यम से स्कीम का
मीधडया द्वारा व्यापक प्रचार दकया जाएगा और व्यधिगत सूचना दी जाएगी, धजससे दक स्कीम के
फ़ायदाग्रधहयों को स्कीम के अधीन ‘आधार’ की आवश्यकता के बारे में जागरूक बनाया जा सके और उनको
सलाह दी जायेगी दक, वे यदद पहल े से नामांदकत नह ह ैं तो, 31/08/2017 तक अपन े क्षेत्र म ें उपलब्ध
धनकटतम आधार नामांकन केद्रों पर जाकर अपना नामांकन करवाएुँ। स्थानीय रूप स े उपलब्ध आधार
नामांकन केद्रों की सूची उ ह ेंउपलब्ध करवाई जाएगी।
(2) यदद स्कीम के अधीन फ़ायदाग्राही, ब्लॉक या तालुका या तहसील जैसे धनकटतम क्षेत्र म ें नामांकन
केंद्र की अनुपलब्धता के कारण, आधार नामांकन करान े में असमथ ा हों तो, रा्य सरकार अथवा
संघ रा्य क्षत्रे प्रिासन का स्वास््य एव ं पररवार कल्याण धवभाग सुधवधाजनक अवस्थानों पर
आधार नामांकन सुधवधा उपलब्ध कराएगा तथा स्कीम के फ़ायदाग्राही अपना नाम, पता, मोबाइलII (ii) 3
नंबर और अय अपेधक्षत ब्यौरे देकर आधार नामांकन हते ु अपना आवेदन रा्य सरकार अथवा संघ
रा्य क्षत्रे प्रिासन के स्वास््य एवं पररवार कल्याण धवभाग के पदनाधमत अधधकाररयों के पास
अथवा सरकारी स्वास््य सुधवधा-केंद्रों और प्राइवेट प्रत्याधयत स्वास््य सुधवधा-केद्रों पर अथवा इस
प्रयोजनाथा उपलब्ध वेब पोटला के माध्यम स े अपन े आधार नामांकन के अनरु ोध को रधजस्टर कराए।ं
3. यह अधधसूचना असम, मघे ालय तथा जम्म-ू कश्मीर रा्य को ोड़कर सभी रा्यों और संघ रा्य क्षेत्र प्रिासनों म ें राजपत्र
में इसके प्रकािन की तारीख़ स ेप्रभावी होगी।

NOTIFICATION
New Delhi, the 16th June, 2017
S.O. 1935(E).—Whereas, the use of Aadhaar as identity document for delivery of services or benefits
or subsidies simplifies the Government delivery processes, brings in transparency and efficiency and enables
beneficiaries to get their entitlement directly in a convenient and seamless manner and Aadhaar obviates the
need for producing multiple documents to prove one’s identity;
And whereas, in the Ministry of Health and Family Welfare the Central Government is administering
the Centrally Sponsored Scheme of Revised National Tuberculosis Control Programme (RNTCP) using a
web based application called Nikshay (hereinafter referred to as the Scheme) under the National Health
Mission for promotion of universal access to tuberculosis (TB) care with early diagnosis and regular treatment
completion. The Scheme is implemented by the State Governments and Union territory Administrations
through the government health facilities and registered private health facilities spread across the country;
And whereas, under the Scheme conditional cash assistance (hereinafter referred to as the benefit) is
offered to the eligible TB patients, private health care providers and treatment supporters (hereinafter together
referred to as the beneficiaries), as per the Scheme guidelines;
And whereas, the aforesaid Scheme involves recurring expenditure incurred from the Consolidated
Fund of India;
Now, therefore, in pursuance of the provisions of the section 7 of the Aadhaar (Targeted Delivery of
Financial and Other Subsidies, Benefits and Services) Act, 2016 (18 of 2016) (hereinafter referred to as the
said Act), the Central Government in the Ministry of Health and Family Welfare hereby notifies the
following, namely: -
1. (1) An individual eligible to receive the benefit under the Scheme is, hereby, required to furnish
proof of possession of Aadhaar number or undergo Aadhaar authentication.
(2) Any individual desirous of availing the benefit under the Scheme, who does not possess Aadhaar
number or has not yet enrolled for Aadhaar, shall have to apply for Aadhaar enrolment by 31/08/2017,
provided he or she is entitled to obtain Aadhaar as per the provisions of section 3 of the said Act and
such person may visit any Aadhaar enrolment centre (list available at Unique Identification Authority
of India (UIDAI) website www.uidai.gov.in) for Aadhaar enrolment.
(3) As per regulation 12 of the Aadhaar (Enrolment and Update) Regulations, 2016, the Department
of Health and Family Welfare, which is responsible for implementation of the Scheme in the State
Government or Union territory Administration is required to offer Aadhaar enrolment facilities for the
beneficiaries who are not yet enrolled for Aadhaar and in case, there is no Aadhaar enrolment centre
located in the respective Block or Taluka or Tehsil, the Department responsible for implementation of
the Scheme in the State Government or Union Territory Administration shall provide Aadhaar4 THE GAZETTE OF INDIA : EXTRAORDINARY [PART II—SEC. 3(ii)]
enrolment facilities at convenient locations in coordination with the existing Registrars of UIDAI or
by becoming UIDAI Registrar;
Provided that till the Aadhaar is assigned to the individual, benefits under the Scheme shall be given
to such individuals subject to the production of the following documents, namely:-
(a) (i) if he or she has enrolled, his or her Aadhaar Enrolment ID slip; or
(ii) a copy of his or her request made for Aadhaar enrolment, as specified in sub-paragraph (2)
of paragraph 2 below; and
(b) (i) Voter identity card issued by the Election Commission of India; or (ii) Permanent Account
Number Card issued by Income tax Department; or (iii) Passport; or (iv) Driving Licence issued
by the Licencing Authority under the Motor Vehicle Act, 1988 (59 of 1988); or (v) Certificate of
Identity having photo issued by a Gazetted Officer or a Tehsildar on official letter head; or (vi)
Address card having name and photo issued by the Department of Posts; or (vii) Bank Photo
Passbook; or (viii) Kisan Photo Passbook; or (ix) Mahatma Gandhi National Rural Employment
Guarantee Scheme Card; or (x) Ration Card; or (xi) Employee Photo Identity Card issued by the
Government or Public Sector Undertakings; or (xii) Any other document as specified by the State
Government or Union territory Administration:
Provided further that the above documents shall be checked by an officer specifically designated
by the State Government or Union territory Administration responsible for that purpose.
2. In order to provide convenient and hassle free benefit under the Scheme, the Department of Health
and Family Welfare in the State Government or Union territory Administration responsible for
implementation of the Scheme shall make all the required arrangements including the following, namely:-
(1) Wide publicity through media and individual notices shall be given to the beneficiaries through the
Government health facilities and private accredited health facilities, to make them aware of the
requirement of Aadhaar under the Scheme and they may be advised to get themselves enrolled for
Aadhaar at the nearest enrolment centres available in their areas by 31/08/2017, in case they are not
already enrolled. The list of locally available Aadhaar enrolment centres shall be made available to
them.
(2) In case, the beneficiaries under the Scheme are not able to enrol for Aadhaar due to non-availability of
the Aadhaar enrolment centres in the vicinity such as Block or Taluka or Tehsil, the Department of
Health and Family Welfare in the State Government or Union territory Administration shall provide
Aadhaar enrolment facilities at convenient locations and the beneficiaries under the Scheme may
register their requests for Aadhaar enrolment by giving their names, addresses, mobile numbers and
other required details with the designated officials of the Department of Health and Family Welfare in
the State Government or Union territory Administration or at Government health facilities or private
accredited Health facilities or through the web portal provided for the purpose.
3. This notification shall come into effect from the date of its publication in the Official Gazette in all
the States and Union territory Administrations except the States of Assam, Meghalaya and the State of Jammu
and Kashmir.
