From Sri E.Sankar S/O late Smt V.Ramalakshmi, Jamedar,application dated 28.01.2012 ***** O R D E R: Sri E.Sankar S/O Late Smt V.Ramalakshmi, Jamedar, Irrigation & CAD (IW) Department in his application 6th cited read above, has requested to settle the claim in respect of Group Insurance Scheme contributed by his mother Late Smt V.Ramalakshmi, Jamedar who has expired while in service on 22.11.2011 at an early date. He has furnished the death certificate and legal heir certificate. 2. Accordingly in terms of the orders issued in G.Os. 1st to 3rd read above, sanction is accorded for payment of an amount of Rs.15,000/- (Rupees Fifteen thousand only) towards insurance fund under A.P.State Employees Group Insurance Scheme to Sri E.Sankar and other eligible legal heirs of Late Smt V.Ramalakshmi, Jamedar of this Department. 3. The amount sanctioned in para-2 above shall be debited to the following Head of Account. “8011 - Insurance and Pension Funds 107 – State Govt. Insurance Fund 01 – A.P.State Employees Group Insurance Scheme 001 - Insurance fund on the event of death 4. Sanction is also accorded for the refund of an amount of Rs.8,454/- (Rupees Eight thousand four hundred and fiftyfour only) in lumpsum being the amount contributed under A.P.State Employees Group Insurance Scheme ( i.e.Rs. 2,654/- Savings fund + 5,800/- interest) to Sri E.Sankar and other eligible legal heirs of Late Smt V.Ramalakshmi, Jamedar, Irrigation & CAD Department as calculated in the statement enclosed to this order. Certified that she has not availed any EOL during the period for which amount is claimed. 5. The amount sanctioned above shall be debited to the following Head of Account : “8011 - Insurance and Pension Funds 107 – State Govt. Employees Insurance Fund 01 – Group Insurance Scheme 002 - Savings fund Interest 6. The Irrigation & CAD (IW-OP:Claims) Department are requested to draw and disburse the amount sanctioned in para-2 & 4 above and disburse to the individual mentioned therein.
