Dept., application along with Medical bills dt: 02.12.2011.

3. From the Civil Surgeon Specialist, Hyderabad, Lr.Rc.No.32/
CDS/2012, dt:01.02.2012.

ORDER:
Sanction is hereby accorded for payment of an amount of Rs.399/-
(Rupees Three hundred and ninety nine only) to Sri A. Dayakar Reddy, the then
Joint Secretary to Government towards reimbursement of Medical Expenses
incurred by him for his treatment of “HTN & General Weakness” during the
period from 12.12.2011 at Government Dispensary, A.P. Secretariat, Hyderabad.
2. The expenditure sanctioned in para (1) above shall be debited to the
following Head of Account:
“3451 - Secretariat Economic Services
090 - Secretariat.
07 - Industries and Commerce Department.
010 - Salaries
017 - Medical Reimbursement.
3. The Industries and Commerce (OP.II Claims) Department shall draw and
disburse the above amount to the individual.
4. This order does not require the concurrence of H.M & F.W. Department as
per the orders in force.
