From the Civil Surgeon Specialist Secretariat Dispensary, LrNo.184/CDS/ 12, dt:28-2-2012 -0o0- O R D E R: Smt.K.Lalitha, section Officer in her application 3rd read above, has requested for sanction of medical reimbursement claim of Rs42,692/- towards medical expenses incurred in connection with the treatment for CAD: Dysfunction, Chonic, Bronchitis IP Hepatitis of her Husband Sri. D,Rama Murthy in Yashoda Super Speciality Hospital, Malakpet, Hyderabad. 2. The Civil Surgeon Specialist, Secretariat Dispensary, Hyderabad to whom the bills have been referred to for scrutiny has reported in his letter 5th read above, that an amount of Rs31747/- is admissible for sanction 3. Therefore, sanction is hereby accorded for an amount of Rs.31,747 /- (Rupees thirty four thousand Seven hundred and forty seven only) to Smt.K.Lalitha, section Officer towards medical expenses incurred in connection with medical treatment of CAD: Dysfunction, Chonic, Bronchitis IP Hepatitis of her Husband Sri. D,Rama Murthy in Yashoda Super Speciality Hospital, Malakpet, Hyderabad. From27-10-2011 to 31-10- 2012 4. The expenditure sanctioned at Para (3) above, shall be debited to “2251- Secretariat – Social Services – (090) –Secretariat – (04) – Higher Education Department 010 Salaries – 017 Medical expenditure”. 5. The Higher Education (OP Claims) Department is requested to draw and disburse the amount to the individual. 6. This order does not require the concurrence Finance Department as per the existing rules in force.
