5. From the Civil Surgeon Specialist, Hyderabad, Lr.Rc.No.219/
CDS/12, dt:31.01.2012.

ORDER:
Government hereby accord sanction for payment of an amount of
Rs.2222/- (Rupees Two thousand two hundred and twenty two only) to Smt. C.R.
Seethamma, Jamedar (Retd.), Industries & Commerce Department towards the
reimbursement of Medical expenses incurred by her for Purchasing Medicines for
her treatment of “D.M., HTN”, during the period from 08.09.2011 to two months.
2. The expenditure sanctioned in para(1) above shall be debited to the
following Head of Accounts:
“3451 - Secretariat Economic Services
090 - Secretariat.
07 - Industries and Commerce Department.
010 - Salaries
017 - Medical Reimbursement.
3. The Industries and Commerce (OP.II Claims) Department shall draw and
disburse the above amount to the individual.
4. This order does not require the concurrence of H.M & F.W. Department as
per the orders in force.
