3. From the Civil Surgeon Specialist, Hyd., Rc.No.2417/CDS/12,
dt:20.01.2012.

ORDER:
Sanction is hereby accorded for payment of an amount of Rs.2357/-
(Rupees Two thousand three hundred and fifty seven only) to Sri Mohd. Ibrahim,
Asst. (Retd.), Industries & Commerce Department for the reimbursement of
Medical Expenses incurred by him for his treatment of “Ischemic Heart Disease”
for one month period from 20.07.2011 to 17.09.2011 at Yashoda Hospital,
Malakpet, Hyderabad.
2. The expenditure sanctioned in para (1) above shall be debited to the
following Head of Accounts:
“3451 - Secretariat Economic Services
090 - Secretariat.
07 - Industries and Commerce Department.
010 - Salaries
017 - Medical Reimbursement.
3. The Industries and Commerce (OP.II) Department shall draw and disburse
the above amount to the individual.
4. This order does not require the concurrence of H.M & F.W. Department as
per the orders in force.
