The substance of the imputations of misconduct or misbehaviour in respect of which the inquiry is proposed to be held is set out in the enclosed statement of articles of charges (Annexure-I). A statement of imputations of misconduct or misbehaviour in support of each article of charge is enclosed (Annexure-II). A list of documents by which, and a list of witnesses by whom, the articles of charges are proposed to be sustained are also enclosed (Annexure – III & IV). 3. Dr. V.Chakrapani, CSS, is directed to submit a written statement of his defence within 15 days of the receipt of this orders. 4. Dr. V.Chakrapani, Civil Surgeon Specialist, is informed that an inquiry will be held only in respect of those articles of charge as are not admitted. He should, therefore, specifically admit or deny each article of charge. 5. Dr. V.Chakrapani, CSS, is further informed that if he does not submit his written statement of defence on or before the date specified in para-3 above, further action will be processed based on the material available. 6. Attention of Dr.V.Chakrapani, is invited to Rule 24 of the A.P.C.S.(Conduct) Rules, 1964, under which no Government servant shall bring or attempt to bring any political or outside influence to bear upon any superior authority to further his interests in respect of matters pertaining to his service under the Government. If any representation is received on his behalf from another person in respect of any matter dealt with in these proceedings it will be presumed that Dr.V.Chakrapani is aware of such a representation and that it has been made at his instance and action will be taken against him for violation of Rule 24 of the A.P.C.S. Chakrapani, CSS, ESI Hospital, Ramachandrapuram has been transferred to ESI Hospital, Rajahmundry, E.G. Dist. on Administrative grounds / Disciplinary grounds and he was deemed to have been relieved on 01.07.2011. But he has not reported for duty at ESI Hospital, Rajahmundry, E.G. Dist. sofar. Thus he disobeyed the orders of his higher authorities and violated Rule -3 of APCS (Conduct) Rules, 1964 and rendered himself liable for disciplinary action. ARTICLE –II : Dr. V. Chakrapani, CSS, ESI Hospital, Ramachandrapuram (under orders of transferred to ESI Hospital, Rajahmundry, E.G. Dist.) has failed to join duty without obeying the Government Orders and has applied leave along with medical certificates with diagnosis of “Cervical Spondilitis with vertibro Basilar In Sufficiency and Brachial neuralgia”. Which will not prevent him to join in the place of transfer i.e. ESI Hospital, Rajahmundry as Government orders and further extending his leave without joining in the place of posting which is causing hindrance in providing the services of the Physician at ESI Hospital, Rajamundry. ARTICLE – III : Dr.V.S. Chakrapani, CSS, while working the Medical Superintendent, ESI Hospital, RC Puram, Medak Dist. has issued the purchase order to M/s Setwin, Hyderabad for purchase of the following items: 1. A.C. (02 Ton) with stabilizer - 03 No’s 2. Wall Clocks - 10 No’s 3. Air Cooler (Desert size) - 08 No’s 4. Zero–B Water Purifier (5 Ltrs.) - 04 No’s 5. Zero-B Water Purifier (10 Ltrs.) – 04 No’s 6. LCD Computer Lap top - 10 No’s As per the letter of the present Medical Superintendent, ESI Hospital, RC Puram the above items have not been received till to date. After completion of leave period he has been extending the Medical Leave from time to time. Article – II : He has failed to join duty without obeying the Government Orders and extended the medical leave from time to time causing hindrance in providing services of the Physician at ESI Hospital, Rajamundry, E.G. Dist. Article – III: Stock entries were already made by the Dy.C.S. RMO –II duly signed and stamped on the back side of the bills and the same was sent to DIMS office for settlement of the bills.
