2. The Essentiality Certificate issued by the Medical Superintendent,
NIMS, Hyderabad dated nil.
3. From Sri. M.V.Suryanarayana, Asst. Section Officer, Industries &
Commerce Department, application along with Medical bills

ORDER:
In pursuance of orders issued read 1st above, sanction is hereby accorded
for payment of an amount of Rs.8,397/-(Rupees Eight thousand three hundred
and ninety seven only) to Sri. M.V.Suryanarayana, Asst. Section Officer,
Industries & Commerce Department towards reimbursement of Medical
Expenses incurred by him for his treatment of Double Vessel Coronary Artery
Disease, CABG during the period from 20/09/2011 to 19/01/2011 at Nizam’s
Institute of Medical Sciences, Hyderabad.
2. The expenditure sanctioned in para (1) above shall be debited to the
following Head of Account:
“3451 - Secretariat Economic Services
090 - Secretariat.
07 - Industries and Commerce Department.
010 - Salaries
017 - Medical Reimbursement.
3. The Industries and Commerce (OP.II Claims) Department shall draw and
disburse the above amount to the Officer.
4. This order does not require the concurrence of H.M & F.W. Department as
per the orders in force.
