No.2051/CDS/2011, Dt. 30.12.2011. &&& ORDER: In the reference 3rd read above, Smt.P.Vanajakshi wife of Late Sri P.V.Raghava Rao, Deputy Secretary to Government, Retired, Transport, Roads & Buildings Department while enclosing the Medical Bills in original has requested to reimburse medical expenses incurred by her for an amount of Rs.10,000/- towards Retina surgery left eye at Maxivision Eye Hospital, Hyderabad. 2. In the reference 5th read above, the Civil Surgeon Specialist, Civil Dispensary, A.P. Secretariat, Hyderabad has scrutinized the bills and recommended for reimbursement of medical expenditure for an amount of Rs. 10,000/- in respect of Smt.P.Vanajakshi wife of Late Sri P.V.Raghava Rao, Deputy Secretary to Government, Retired, TR&B Dept. 3. Smt P.Vanajakshi wife of Late Sri P.V. Raghava Rao has stated that her husband expired on 3.4.2010 and requested for sanction of medical reimbursement bills in her name. She has furnished Death Certificate and Family Member Certificate issued by Tahsildar, Shaikpet Mandal, Hyderabad. 4. Government, after careful examination of the matter, hereby accord sanction for an amount of Rs. 10,000/- (Rupees Ten Thousand Only) towards reimbursement of medical expenses incurred by her towards undergoing Retina Surgery Left Eye on 01.11.2010 at Maxivision Eye Hospital, Hyderabad. 5. The expenditure shall be debited to the Head of Account of “3451 – Secretariat – Economic Services – 090 – Secretariat 10 – T.R.&B Department - 010 – Salaries – 017 – Medical Reimbursement”. (PTO)::2:: 6. The Transport, Roads and Buildings (O.P.II) Department shall draw and disburse the amount sanctioned in para 4 above to Smt P.Vanajaksh,i wife of Late Sri P.V.
