Yadaiah, Office Subordinate, Dt. Nil. * * * ORDER:- Sri V. Yadaiah, Office Subordinate, LET&F Department, retiring from service on the attaining the age of superannuation on 29.02.2012 has contributed towards Group Insurance ` Rs.10/- per month 11/84 to 10/94 (120 months), @ ` 15/- per month from 11/94 to 02/2012 (208 months) and he has requested the Government to the said amount in the reference 3rd read above. 2. In pursuance of the orders issued in the 1st and 2nd read above sanction is hereby accorded for payment of ` 12,413/- (Rupees Twelve thousand Four hundred and Thirteen only) towards Group Insurance Scheme Fund and Interest there on as shown in the Annexure to this order. 3. The amount sanctioned in para 2 above shall be debited to the following Head of Account:- Particulars Amount Head of Account Contribution ` 3,009-00 8011- Insurance and Pension Fund , MH- of G.I.S. 107-State Govt. Employees GIS, SH-01- Govt. Insurance Scheme, 002-Savings Fund Interest ` 9,404-00 8011- Insurance and Pension Fund , MH- amount on 107-State Govt. Employees GIS, SH-01- Savings Govt. Insurance Scheme, 003-Interest from Fund Government. Total ` 12,413-00 4. The Labour, Employment, Training and Factories (Claims) Department are requested to draw the said sanctioned amount by way bank account of Sri V.
