L.Dis No. 7440(1)MA-A/2012, Dt. 23.2.2012. &&& ORDER: In the reference 3rd read above, Smt D.Sharada wife of Sri D.Subba Raju, Section Officer, Transport, Roads and Buildings Department, while enclosing the medical bills in original, has requested to reimburse medical expenses incurred for his treatment for “Carcinoma in Stomach MR No 89378 ” during the period from 06.09.2011 to 12.09.2011 at M/s Basavatarakam Indo- American Cancer Hospital and Research Institute, Banjara Hills, Hyderabad and incurred an amount of Rs.56,734/- 2. In the reference 6th read above, the Director, Medical Education, Sultan Bazar, Hyderabad, has scrutinized the bills and recommended for reimbursement of medical expenditure for an amount of Rs. 56,684/-,being the net admissible amount of the total amount claimed. 3. Government, after careful examination of the matter, hereby accord sanction for an amount of Rs. 56,684/- (Rupees Fifty Six Thousand Six Hundred And Eighty Four Only) towards the medical reimbursement expenses incurred by Sri D.Subba Raju, Section Officer, Transport, Roads and Buildings Department in connection with his treatment for “Carcinoma in Stomach, MR No 89378 ”during the period from 06.09.2011 to 12.09.2011 at M/s Basavatarakam Indo-American Cancer Hospital and Research Institute, Banjara Hills, Hyderabad. Sri D.Subba Raju, Section Officer, expired on 12.9.2011. 4. In the reference 4th read above, Smt.D.Sharada wife of Late Sri D.Subba Raju, Section Officer, has requested for sanction of medical reimbursement amount in her name. She has submitted Death Certificate, Legal Heirs Certificate issued by Tahsildar, Ameerpet Mandal and No Objection Certificate from her children. 5. The expenditure shall be debited to the Head of Account of “3451 – Secretariat – Economic Services – 090 – Secretariat 10 – T.R.&B Department - 010 – Salaries – 017 – Medical Reimbursement. ” (PTO)::2:: 6. The Transport, Roads and Buildings (O.P.II) Department shall draw and disburse the medical reimbursement amount sanctioned in para (3) above to Smt D.Sharada wife of Late Sri D.Subba Raju, Section Officer, Transport, Roads and Buildings Department Hyderabad ( Account No: 20104813840, State Bank of India, Hyderabad Public School Branch, Hyderabad.) 7.
