Application from Smt.Yadamma, Office Subordinate, Dated:18-2-2012. *** O R D E R: Smt.Yadamma, Office Subordinate, Planning Department retired from Government service on 31-03-2012 and she was relieved of her duties on Afternnon of 31-02-2012 vide reference 4th read above. 2. In pursuance of the orders issued in the Government orders 1st and 2nd read above, Smt.Yadamma, Office Subordinate (Retired) while in service has contributed at the rate of Rs.10/- per month with effect from 1-11-1984 to 31-10-1994 (i.e., 10 years) and the rate of Rs.15/- per month with effect from 1-11-1994 to 31-03-2012 to the scheme every month. The total contribution with the interest thereon under Group Insurance Scheme made by her is Rs.12,506/-. 3. Consequent on her retirement, sanction is hereby accorded for payment of Rs.12,506/- (Rupees Twelve thousand five hundred and six only) being the Group Insurance Scheme Savings Fund together with interest accrued thereon to Smt Yadamma, Office Subordinate (Retired), Planning Department. Necessary entries have been made in her Service Register. 4. The expenditure sanctioned in para (3) above shall be debited to the Head of Account “8011 – Insurance and Pension Fund – M.H.(107) –A.P. State Government Employees Group Insurance Scheme – S.H.(01) – Group Insurance Scheme (002) Savings Fund – (003) Interest from Government. 5. It is certified that Smt.Yadamma, Office Subordinate, Planning Department has not gone on E.O.L. during the period of contribution till her retirement. 6. The planning (Claims) Department shall draw and disburse the Group Insurance Scheme Fund amount to Smt.Yadamma, Office Subordinate, Planning Department. P.T.O:: 2 :: 7. In future, if it is found that any excess mount was paid, the same will be deducted from her pensionery benefits without any notice. 8. This order does not require the concurrence of Finance (Pensions) Department as per the orders in force.
