From the Civil Surgeon, Government Civil Dispensary, Secretariat, Hyderabad, Lr.Rc.No.498/CDS/Hyd/2012, dt.3-3-2012. **** O R D E R: In the reference 1st read above, Sri Sanjay Jaju, Secretary to Government, IT & C Department, has furnished medical bills amounting to Rs.21574 along with Prescription, Essentiality Certificate, and Appendix-II in original for treatment to his father at Government Civil Dispensary, A.P.Secretariat, Hyderabad and requested to reimburse the same. 2. The Civil Surgeon, Government Civil Dispensary, Secretariat who has been requested to scrutinize the medical bills in the Government memo 2nd read above, has informed in his letter 3rd read above that, after scrutiny of the medical bills, the net admissible amount for reimbursement is Rs.20623/-. 3. Accordingly, Sri Sanjay Jaju, Secretary to Government, IT & C Department is hereby sanctioned an amount of Rs.20623/- (Rupees twenty thousand six hundred and twenty three only) towards reimbursement of medical expenses incurred by him towards treatment to his father at Government Civil Dispensary, A.P.Secretariat, Hyderabad.:2: 4. The above expenditure shall be debited to “3451 – Secretariat Economic Services, MH 090 – Secretariat, SH (22) – IT&C Department, 010 – Salaries, 017-Medical Reimbursement”. 5. The Joint Director and Drawing & Disbursing Officer, IT&C Department shall draw the amount sanctioned in para 3 above and disburse the amount to the individual.
