Secretariat, Hyderabad Lr.No:295/CDS/2012, dt:04.02.2012. @@@@ ORDER: Sri M.R.Praveen Kumar, Record Assistant, School Education Department in his application 4th read above, has requested the Government to reimburse an amount Rs.27,684/- incurred towards the Medical expenses for his Mother’s treatment for “Phaco IOL surgery for the Right eye” on 21.01.2012 at L.V.Prasad Eye Institute, Hyderabad. 2. In the reference 6th read above, the Civil Surgeon Specialist, Govt.Civil Dispensary, A.P. Secretariat, Hyderabad has scrutinized the Medical Bills and informed that an amount of Rs.14,000/- (Rupees Fourteen Thousand only) is admissible for reimbursement. 3. Government, after scrutiny of the claim in terms of G.Os 1st, 2nd & 3rd read above, hereby accord sanction for reimbursement of an amount of Rs.14,000/- (Rupees Fourteen Thousand only) to Sri M.R.Praveen Kumar, Record Assistant, School Education Department incurred towards the Medical expenses for his Mother’s treatment for “Phaco IOL surgery for the Right eye” on 21.01.2012 at L.V.Prasad Eye Institute, Hyderabad.. 4. The expenditure sanctioned in para (3) above shall be debited to “2251 – Secretariat – Social Services – 090 Secretariat SH(18) – Education (SE-Wing) Department, 010 Salaries – 017 Medical Reimbursement” 5. The Education (SE.OP/Claims) Department are requested to draw the amount sanctioned at para (3) above and credit the same into the individual’s bank account through ECS. 6.
