NOTIFICATION
New Delhi, the 21st December, 2015
S.O. 130(E).— Whereas M/s Jet Airways B737-800 aircraft VT-JFA met with a serious incident at
Trivandrum on 18.08.2015 while operating flight 9W-555 (Doha-Cochin).
And whereas, it appears to the Central Government that it is expedient to investigate and determine the causes
and contributory factors leading to the said serious incident and make recommendations to avoid recurrence of such
serious incidents in future.
Now, in the exercise of the powers conferred by the Rules 11(1) of the Aircraft (Investigation of Accidents
and Incidents) Rules, 2012, the Central Government hereby directs that a formal inquiry in the serious incident be
made.
Accordingly, the Central Government hereby appoints a Committee of Inquiry headed by Shri A X Joseph,
Deputy Director, AAIB with following members:
(i) Shri Jasbir Singh Larhga, Assistant Director, AAIB as Secretary (to associate with the committee for
purpose of on job training)
(ii) Capt. Sharad Dogra, as member
The committee will also take the assistance of other experts/agencies whenever required.
The headquarters of the Committee will be at New Delhi.
The Committee will complete its inquiry and submit its report to the Central Government preferably within six
months.
